JOB PURPOSE OR MISSION: The Licensed Practical Nurse’s (LPN) primary role is to provide nursing care for patients who are experiencing common, well-defined health problems. The LPN functions as a member of the multidisciplinary health care team, and utilizes the nursing process in implementation of patient’s care. Those services are performed under the supervision of a registered nurse and utilize standardized procedures with predictable outcomes. The LPN adheres to standards and regulations as set forth by the facility’s policies and procedures as well as external credentialing bodies such as JCAHO, OMH and OSHA. Works closely with nurse navigators and other staff to meet the needs of clients. Serves as a liaison between nursing staff and physicians and other health care practitioners, schedules the patient for face to face visits to ensure appropriate certification of terminal illness, arranges for needed medications/DME/supplies, and obtains orders as needed.
