Position: Float Advanced Practice Provider (NP/PA) Monogram Health is seeking compassionate and skilled Advanced Practice Providers (Nurse Practitioners or Physician Assistants) to deliver high‑quality in‑home care to patients living with multiple chronic conditions, including diabetes, hypertension, chronic kidney disease, heart failure, COPD, depression, and other complex health needs. Job Summary and Responsibilities As a Certified Medical Assistant (CMA), you will be a vital pillar of our care team, significantly impacting patient well-being through direct support and seamless coordination. Every day, you will actively collaborate on comprehensive screenings, assist with clinical procedures, provide essential health education, and expertly manage administrative tasks, leveraging technology to coordinate care for diverse patient populations. You will ensure patients feel at ease, patiently explaining instructions and addressing their concerns. To be successful in this role, you will possess strong clinical knowledge, technological fluency, genuine compassion for patient well-being, and an adaptable spirit keen on continuous learning and collaborative teamwork. Perform general patient care by following established standards and procedures. Greet and prepare patients for the health care provider.
