Pay Range: $24.02 - $31.22 Scheduled Weekly Hours: 40 Position Overview ESSENTIAL FUNCTIONS Interrogates pacemakers and ICDs, monitor remote access of devices: interrogates/reprograms and troubleshoots patients with implantable pacemakers, ICDs and loop recorders. Coordinates treatment and care for patients with devices. Interacts with clients, patients and Practice staff, providing patient care, communicating, organizing and resolving issues with regard to monitoring. Presents patients to the doctor in clinic. Alerts doctor of any problems with patient or device. Ensures accurate recording (saving to program), reporting (uploading, creating letters from template for physician review) and authorization (physician signature); Other data entry includes billing codes. Coordinates clinic scheduling needs, including coordination of device tech vendors and representatives. Enrolls patients electronically for remote monitoring and educate patients on device care and follow up including home monitoring instructions. Provides proper support as medical assistant as necessary (prepare and room patient, take vitals/bp, conducts EKG or ECT, review meds). May perform INR for Coumadin and refill scripts, assist with clinical calls from patients, physician offices, hospitals and nursing homes. Handles nursing emails sent from offsite remote monitoring company. Based on level of education and certification may complete treadmill stress tests; CVT: Complete non-invasive peripheral vascular tests by collecting ultrasound echoes. Maintains equipment/systems, follow operating instructions; report hazardous conditions; troubleshoots calibrate equipment; follows preventive maintenance procedures; anticipates repair requirements; calls for repairs. Promotes positive patient relations. Assures confidentiality of all information related to patients. Organizes priorities and works independently and as a member of a team to complete tasks. Supports team to ensure delivery of excellent patient care by assisting with and assuming diversified tasks as required. Demonstrates flexibility and cooperation in relation to workplace staffing and manpower needs by rotating as needed among various office locations. Performs other duties as needed. MINIMUM REQUIREMENTS High school diploma or equivalent. Current certification in CPR Basic or Advanced Life Support (BLS or ACLS). License/registry to perform CRAT (Certified Rhythm Analysis Technician) required. Graduate of medical assistant certification program preferred. Two years previous clinical experience in cardiac setting required. Previous experience with heart monitoring devices preferred. Shift Type: Per Diem Compensation: $70/visit for initial assessment and $60/visit for 60-day visits Visiting Angels is seeking a Home Care Field Registered Nurse (RN)/Case Manager for in-home visits to our clients throughout Mercer County and ideally some Burlington County clients, as well (optional). This is a non-medical per diem position There are no set hours for this position Our Home Care Field Registered Nurses (RNs) prepare thorough and accurate Initial Assessments and Reassessments and develop personalized care plans for our clients. This part-time/per diem position is ideal for an RN who desires a significant level of work but with flexible hours. Most visits to our clients occur on weekdays. PURPOSE AND SCOPE: Functions as the hemodialysis team leader in the provision of chronic hemodialysis care and treatment. Provides day to day direction and supervision to assigned direct patient care staff. Communicates with the physician and other members of the healthcare team to interpret, adjust, and complete patient care plans. Provides safe, effective delivery of patient care in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the company, state, and federal agencies. PRINCIPAL DUTIES AND RESPONSIBILITIES: General and Staff Related: Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participate in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommend disciplinary action to Clinical Manager and initiate as appropriate. Continuously monitor patient's condition with regards to problems and potential complications associated with dialysis. Administer medications to patients per physician's orders. Act as the subject matter expert and as a resource for staff members. Supervise and participate in completion of short- and long-term care plans. Admit new patients according to facility procedure. Ensure educational needs of patients are met and educate the patient and family about End Stage Renal Disease, dialysis therapy, diet and medication. Technical: Supervise the safe and effective use of all equipment involved in direct patient care. Operate all dialysis related and emergency equipment safely and efficiently when needed. Perform required testing and verification and initial the checklist for start-up and shut-down procedures as outlined in the Technical Services Manual. Complete Nurse's Technical Training Program/Water Quality Facility Training. Other: Assist with special projects or other duties as assigned by the Clinical Manager Assist with the interviewing of potential direct patient care staff as requested. Promote efficient use of medical supplies. Attend and participate in monthly Quality Assurance meetings. Other duties as assigned.
