Overview Newport Health Center is currently seeking a primary care Advanced Practice Registered Nurse or Physician Assistant to work with our team to manage the health and wellness of our patients. You will work collaboratively to provide comprehensive care to our patients in western New Hampshire. Working under the general direction of the medical director, you will practice autonomously as well as in collaboration with physicians, APRNs, and PA colleagues as well as other healthcare providers to provide primary healthcare to patients in our practice. Overview Care Manager - Regional Float, System Staffing Office Schedule varies, Days including rotation to weekend Regularly scheduled min (1x12 hour shift p/week) One would float to all six D-H system hospitals. In this role you will have exposure to and work in the D-H Lebanon Main Campus, and the member hospitals such as Alice Peck Day in Lebanon NH, Valley Regional in Claremont NH, New London Hospital in New London NH, Mount Ascutney in Vermont, and Cheshire Medical Center in Keene NH. Identifies gaps in the care continuum and work with the community and provider networks to expand access to needed clinical, psychological, social and environmental services. Participates in the development of clinical disease management strategies and identifies the appropriate measures for the evaluation of outcomes. L4WD Inpatient Surgery is a collaborative unit that cares for complex surgical patients. Home to thoracic, vascular, kidney transplant and general surgery patients (typically either laparoscopic or major abdominal procedures), the population is diverse and challenging. Our nurse-to-patient ratios are very attractive and Licensed Nursing Assistants are also part of our collaborative team. As a member of the L4WD team you will assess, plan, organize, provide and evaluate nursing care utilizing the nursing process. In addition, you will make nursing judgments and decisions and will be responsible for nursing care and documentation. Overview Responsible for clinical duties such as taking vital signs, giving injections, performing simple diagnostic tests, collecting specimens, drawing blood, sterilizing and cleaning equipment, and maintaining examination rooms at an outpatient care site. Overview The Section of Medical Oncology at Dartmouth-Hitchcock Medical Center is seeking an experienced Breast Oncology Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) to provide outpatient healthcare to individuals within the Comprehensive Breast Oncology Program of the Dartmouth Cancer Center. The successful candidate will assess, treat and manage the healthcare needs and problems of patients and families within their specialty area(s). The APRN/PA functions as part of the provider team to provide care to the department’s panel of breast oncology patients. This position is full-time, 40 hours per week. Responsibilities Assists with routine patient care activities under the supervision of the Registered Nurse, including, but not be limited to, performing ADL’s, obtaining clinical data, including vital signs, heights and weights, utilizing universal precautions, collecting specimens or performing point of care testing and recording results in medical record, etc. Assists in maintaining a clean and safe work environment by keeping the hallways clear of excess equipment, emptying linen bags when full, etc. Assists visitors and answer telephones. Assists new co-workers in learning the work of the department. Performs other duties as required or assigned. Qualifications HS/GED strongly preferred but not required Must demonstrate keyboarding skills and be familiar with computer use. Previous hospital experience preferred. Required Licensure/Certifications Licensed Nursing Assistant in the State of NH required. Basic Life Support (BLS) Certificate required.
