Job Summary The Integra Transition Manager collaborates with a group of Physicians to pursue cost-effective, quality focused management interventions by achieving a predetermined financial and clinical outcome. Prepare patient chart and deliver to Medial Records upon discharge of patient, or to the Nursing office upon expiration of the patient. Maintain up-to-date records on computer for Bed Management. Access lab results from the computer for the physician. File documents following standard procedures. Give enemas, reinforce simple non-sterile dressings, apply binders, collect specimens, apply heat or cold devices and perform other routine tasks. Assist the RN or LPN in caring for seriously ill patients or in complicated treatments or procedures. Maintain current knowledge of specialty by attending seminars, in-services and reading journals.
