Dialysis Treatment: · Provides safe and effective delivery of care to patients with ESRD. · Accurately implements treatment prescriptions including Sodium (Na) modeling prescription, and Ultrafiltration modeling (where appropriate) to ensure stable treatment therapy as indicated. · Assesses patients’ response to hemodialysis treatment therapy, making appropriate adjustments and modifications to the treatment plan as indicated by the prescribing physician. Communicates problems or concerns to the Team Leader or physician. · Identifies and communicates patient related issues to Team Leader or physician. · Initiates Initial and Annual Nursing Assessment, and ongoing evaluation and documentation of patient care needs according to FMC Policies and Procedures. · Actively participates in the pre-evaluation, initiation, monitoring, termination, access homeostasis, and post evaluation of patients receiving hemodialysis treatment therapy according to established FMC procedures. · Takes appropriate intervention for changes in patient adequacy status and troubleshooting access flow issues as identified by OLC/AMP yellow lights. · Provides, supervises (if applicable), and monitors hemodialysis access care according to established procedures. · Implements, administers, monitors, and documents patient's response to prescribed interdialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. · Ensure accurate and complete documentation by Patient Care Technician on the Hemodialysis Treatment Sheet. Laboratory-related: · Reviews, transcribes, and enters physician lab orders accurately into the Medical Information System. · Ensures appropriate preparation of lab requisitions for Spectra or alternate lab. · Ensures correct labs tubes are utilized for prescribed lab specimens and that lab draw and processing procedures are performed appropriately for all lab samples. · Identifies and ensures appropriate follow-through regarding missed labs and specimens reported to be insufficient according to company policies and procedures. · Ensures all specimens are appropriately packaged according to Department of Transportation (DOT) policies and procedures relating to shipment of blood or body fluid specimens and potentially hazardous material. · Ensures that all labs are directed and delivered to appropriate labs. · Reports alert/panic and abnormal labs result to appropriate physician. · Ensures lab results are forwarded to physicians as requested. MEDICAL RECORDS & DOCUMENTATION: General · Ensures all relevant data including physician orders, lab results, vital signs and treatment parameters, and patient status are documented appropriately and entered into Medical Information System. · Ensures all appropriate patient-related treatment data is entered into the Medical Information System. · Ensures all FMCNA policies regarding patient admission, transfer, and discharge are appropriately implemented. · Ensures and verify accuracy of Patient Care Technician documentation. Daily · Reviews and ensures appropriate daily completion of Hemodialysis Treatment Sheets by all patient care staff. · Ensures that all appropriate procedures are followed regarding opening and closing procedures, inclusive of monitoring that all staff and patients have safely left the premises. Monthly · Initiates, documents, and completes ongoing Continuous Quality Improvement (CQI) activities including monthly reports. · Completes monthly nurses' progress note. · Ensure patient medical records are complete with appropriate information, documentation, and identification on each page (Addressograph label is on all chart forms). · Reviews transplant status and follow established procedure regarding appropriate action to be taken. · Completes patient care plans for new patients within the initial 30 days or any patients deemed unstable requiring monthly patient care plans. · Completes any long-term programs that are due. It is the Unit Manager and Wound nurse’s responsibility to ensure that these skin sheets are being completed by the charge nurse weekly. UM’s need to remind Charge Nurse of the weekly responsibility that day and check the following day for completeness. ON-GOING -Wound Nurses responsibility is to help Prevent wounds. Daily Education to the staff: reminders to T&P residents (in wheelchairs, geri-chairs and in bed), Toileting, incontinent care. This can be done as the Wound Nurse progresses through the day and notices abnormal findings of the resident (dry BM, soaked, wound becoming worse). A Proactive VS Reactive approach to wound care will make the facility successful in wound care management. Holding the Nurses and CNA’s accountable for preventative maintenance. It is a team effort to heal wounds and to not create new wounds. -When wounds become worsened, the treatment should be changed within a 2-week period with the input from the Wound physician and PCP at facility. -The following preventative items should be completed for residents at risk of developing Center Acquired Pressure Ulcers: Orders for T&P, Preventive barrier cream for all residents (Using a barrier cream with Zinc is beneficial in findings), Pressure relieving device to chairs If resident has a stage 4 they need to be on a Roho or something comparable). Every resident in the facility should have a gel cushion at minimum to their wheelchair/Geri chair. Braden assessments completed with all new admissions and re-admissions, quarterly and with any change in condition of the resident. If the Braden scores lower than a specialty mattress should be applied. At risk care plans should be placed for all resident because everyone is at risk. Rounds by UM’s/charge nurses and wound nurse to be performed to ensure that residents are positions appropriately, T&P q 2 hours and prn in bed and wheelchairs/Geri chairs, heels elevated and no skin to skin contact. 1. Consults with Nursing team concerning assessment evaluations and assist in planning an developing the skin care treatment to be performed. 2. Initiate requests for consultation or referral. 3. Asses the residents and their records on admission and discriminate between normal and abnormal findings in order to recognize when to refer the resident to a physician for evaluation, supervision or directions. 4. May act as a Unit Nurse when directed. 5. Provides supervision to C.N.A’s and all subordinate staff which includes checking their work to ascertain that assignments have been completed. 6. Administer medications within the scope of practice of the R.N. or L.P.N. Licensure. Maintain a current report of narcotics received and used. 7. Carry out direct contemporaneous charting in your shift. Chart detailed monthly evaluations of each resident that reflect his/her condition and progress. 8. Ensure that residents care plans are being followed and asses each Guest’s status in accord with their care plan. 9. Round with Wound physician weekly and prn with physicians 10. Must be knowledgeable of individual care plans and support the care planning process by reporting specific information and observations of the resident’s needs, preferences and report any behavioral changes. 11. Understand and follow the “ Six rights of medication administration” and all of Bickford’s policies and procedures regarding medication management. Understand and appropriately utilize the eMAR system for medication administration. Communicate appropriately with clinical leadership for clarification and direction regarding resident medication administration and care. Providing direct resident care Supervision of resident care work of non-licensed team members and other areas of the branch as appropriate. You will be a leader in demonstrating the Bickford Mission ‘Whatever it Takes’ to all other team members on shift. Qualifications: Must have Medication Technician certification for Virginia High School Diploma or GED. Willing to take additional training or education, which specific states may require. CPR and First Aid certifications are required. Ability to handle multiple priorities. Possess written and verbal skills for effective communication. Competent in organizational and time management skills. Demonstrates good judgment, problem-solving, and decision-making skills Ability to utilize or learn Bickford’s eMAR system to administer medications.
