On an Average Day You Will: (includes but not limited to) Perform personal care needs after given a patient's assignment and attend to his/her requests promptly; if unable to perform a certain task, report to the Case Manager immediately Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality Meet safety needs of patient and use equipment safely and properly (foot stools, side rails, O2 etc.) Provide personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned Assist in dressing and undressing patients, as assigned Plan and prepare nutritious meals, including shopping, as assigned Assist in feeding the patient, as assigned Take and record oral, rectal and auxiliary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency Provide proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence Assess and report on patient's condition and significant changes to the Case Manager; also, being aware of the caregiver or other individuals living with the patient and interpersonal issues Assist in ambulation and exercise as instructed by the hospice nurse or therapist Provide normal range of motion and transfers/positioning, as directed Assist with self-administered medications as allowed by state regulations, such as “reminders” Offer and assist with bedpan and urinal aid, as assigned Aid with light laundry needs if necessary Perform range of motion and other simple procedures as an extensional therapy service, as ordered with appropriate completed/demonstrated skills competency Provide respite for patient's family/caregiver when on-site, as appropriate Maintain patient's living area is clean and orderly, as assigned Adhere to the organization's documentation and care procedures and standards of personal and professional conduct Participate in quality assessment performance improvement teams and activities Respect patient and family/caregiver environment and patient's personal needs All other duties as assigned Requirements: Must be a Certified Nursing Assistant ( applicable for all States, additional requirements for California residents, see below). CALIFORNIA REGULATION : Must be a CNA in good standing AND must have a current Home Health Aide (HHA) certification in good standing in the State of California. On an Average Day You Will: (includes, but not limited to) Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children Complete history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documents the findings in the patient's medical record Manage health conditions and terminal illness by developing an individualized plan of care, prescribing medications and/or treatments, obtaining consultations or making referrals and coordinating hospice care services Complete the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations Provide clinical information to the hospice medical director for interpretation of eligibility for hospice recertification Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care Review clinical information, coordinate and oversee patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners Assure that patient receives appropriate measures to control symptoms, through collaboration with interdisciplinary team members Review and develop protocols for treatment and propose options for interventions based on the literature in collaboration with the collaborating physician Consult the collaborating physician when the patient's plan of care is outside standardized practice and protocols Collaborate with the interdisciplinary group to create, review and revise the patient's plan of care Fulfill the obligation of requested and/or accepted assignments Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues Complete all other duties as assigned Requirements: Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program Must have current nursing licensure in State Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience Must have accreditation from an approved certifying body for advanced practice nursing as required by the State Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics) Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order Join a Team that embraces the reverence of life!
