The Team Leader also provides guidance and follow up to the units’ case managers, especially in cases with complex issues. The role is primarily focused on the continuum of care needs of our patients and their families. Close coordination and daily collaboration with the ICM Managers, Medical Director, physicians and other members of the healthcare team is critical. Consistent coaching and mentoring the staff to achieve optimal patient care outcomes is a function of the ICM Team Leader. As a role model, effective communication skills and professionalism are critical. The Team Leader assists the ICM Managers with the identification of opportunities for improvement, case management and social services needs and utilizes research, analysis and evidence based practice. Education: A Bachelor’s degree in Nursing is required. Masters in Nursing or healthcare related field preferred.
