Overview To support the Mission of Helio Health, Inc., the CTI Licensed Practical Nurse will provide comprehensive care management and coordination of services to Critical Time Intervention (CTI) program enrollees. The CTI model is a time-limited form of case management that focuses on helping people with severe mental illness (SMI) and a history of homelessness during a “critical time” transitional period in their lives. CTI Licensed Practical Nurse: CTI is designed specifically to enhance the continuity of care during transition from hospital to community care. In order to support our mission, Helio Health is seeking a Registered Nurse (RN) to work on a Full-Time basis for the Mobile Medication Unit as part of our Utica Opioid Treatment Program providing clinical services to patients in Oneida and surrounding counties. The schedule for this position will be Monday through Friday or Tuesday through Saturday from between the hours of 5:00am - 1:00pm through 6:00am - 2:30pm. Pay Range: $37.00 - $42.00 per hour.
