Job Description The NA - CSR performs various patient care activities and related nonprofessional services necessary in caring for the personal needs and comfort of patients. The NA - CSR assists with patient cleaning, feeding, and preparation for treatment, takes and records vital signs, changes bed linen, prepares treatment trays and other supplies, and assists nursing service personnel with a variety of other patient care duties. The NA - CSR assists the clinical staff by performing a variety of clerical tasks that ensure the efficiency of the unit and provide outstanding customer service to our patients and visitors. Responsibilities Patient Care: This will have been satisfactorily performed when… Unit and patient room and bed are appropriately prepared in an effort to ensure that patients are adequately and properly accommodated during their hospital stay at all times. Patient temperature, pulse, respiration, weight, height, blood pressure, and intake-output measurement is taken and recorded as directed by appropriate personnel in a caring and efficient manner and documents information in CIS. Patients are assisted with daily living activities (e.g., bed bath, oral hygiene, shaving, dressing, back rubs, linen freshening), and provides general care (e.g., positioning patients, pre- and post-operative care, assisting with bed pan use) in a manner that demonstrates compassion and understanding for all patients as evidenced by lack of complaints. Patients are prepared for meals, served food, and fed when needed in a manner that demonstrates compassion and respect for patient needs at all times. Physician orders are accurately transcribed within specified time frames and all necessary forms completed; Communicates information appropriately---including computer/telephone requests for Housekeeping, Linen Services, Plant Operations, Lab, X-ray, EKG, etc. as evidenced by lack of customer complaints Clinical data is accurately recorded within specified timeframes on the patient chart as instructed by the nurse or physician. The Perinatal Nurse Navigator is a nurse based in hospitals or clinics who provides targeted, time-limited support for pregnant and postpartum patients, particularly those with or at risk for substance use disorder (SUD/OUD) during critical transitions in their care. This role facilitates connections from the inpatient or birthing hospital to prenatal, postpartum, and community resources. Additionally, the Navigator works closely with the Project M.O.M. Regional Navigator to provide additional support for these patients.
