The Certified Nursing Assistant is responsible for performing direct student care and student care related activities, including, but not limited to: personal care, activities of daily living, and other clinical skills necessary to meet the objectives stated in the individual student plan of care. Maintains a clean and safe working and/or living environment in the PACE center and/or participants’ homes. Assists with meal and snack preparation, serving, feeding as needed, and clean-up. Uses safe techniques in all interactions with participants at the PACE Center and in participants’ homes. Provides accurate and timely documentation in the EMR and other systems as required by SE policies and procedures and/or as assigned by supervisor. Participates in carrying out schedule of daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines for increased precautions Actively participates in distribution of work for health aides to ensure care needs of participants and site tasks are completed timely and appropriately including but not limited to community-based care and center-based tasks, as assigned Actively participates in regular team meetings for health aides at respective site Participates in carrying out infection control precautions and increased cleaning and disinfecting of the PACE center as assigned and in accordance with current CDC guidelines Performs all duties in accordance with FH and Summit ElderCare policies and procedures. Qualifications Education: Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but we will accept equivalent work experience in lieu of certificate. Certification: Certification as a Home Health Aide or Nursing Assistant is desired - but we will accept equivalent work expereince instead of a certificate - Reliable Transportation Required. Experience: One year of experience working with a frail and/or older adult population, preferably in a PACE program. If under one year, must complete Summit Eldercare training program for health aides prior to working with participants independently. Must complete a standardized set of Summit Eldercare competencies for Health Aides on an annual basis. Pay Range Disclosure: In accordance with the Massachusetts Wage Transparency Act, the pay for this position is $21.50 per hour weekdays - $24.00 per hour weekends which reflects what we reasonably and in good faith expect to pay at the time of posting. Job Description Balance Life & Work with a New Career Opportunity (LONG TERM CARE) (SHORT TERM REHAB (SKILLED NURSING) (LTACH) Now Hiring – Registered Nurse – East Longmeadow, MA $45.00 - $51.00/hour Compensation will be based on, but not limited to, experience, qualifications, credentials, employment status, scheduled shift, and, if applicable, differentials related to a specific shift(s)/and or day(s) CareOne at Redstone Responsibilities The Registered Nurse is responsible for the Shift coordination and oversight of the nursing process for residents assigned to his or her care. Responsibilities include both the direct and indirect provision of care according to each resident's nursing and medical plan of care and in accordance with state and federal regulations and accepted nursing standards of practice. Job duties include, but are not limited to: Provide direct nursing care to assigned residents in accordance with the nursing process. May be assigned to function as Medication/Treatment Nurse, unit Charge Nurse, or Supervisor. Direct day to day functions of Licensed Practical Nurses and Certified Nursing Assistants, assuring compliance with state and federal regulations and facility policies and processes. Provide assessment of and responds to resident conditions utilizing the nursing process. Work cooperatively with other members of the facility interdisciplinary team. Maintain effective communication with other members of the interdisciplinary team utilizing facility processes in order to assure the highest standard of care is maintained for each resident assigned to his or her care. Complete and ensure LPN and C.N.A. documentation of care and services rendered according to facility policies and procedures. Complete reports such as the 24-hour change in condition report, Incident and Accident reports, etc. according to the facility policy and procedure and as assigned by the Director of Nursing Services. Provide immediate response to situations of resident risk such as allegations of abuse, neglect or misappropriation of property. Report incidents timely and to appropriate persons per facility policy. Ensure timely and accurate medication and treatment administration. Communicate to physician and documents changes in resident condition utilizing both objective and subjective nursing assessment. Document and implement changes in nursing and medical plans of care. According to facility policy and procedure, assure that all members of the interdisciplinary team are aware of and understand changes in resident nursing and/or medical plan of care. Position Requirements Associate's Degree from credentialed school of nursing.
