Performs joint home visits with Charge Nurse. Validates diagnoses with patient/family and other health care team members. Documents diagnoses in a manner that facilitates the determination of expected outcomes and plan of care. Initiates Problem List at the time of admission and updates as needed. Incorporates principles of age-specific care into the patient’s plan of care. Documents that the patient’s individualized plan of care has been discussed with and agreed upon by the patient/family. Begins plans for discharge at time of admission; consults with patient/family and health care team members regarding discharge plan. Identifies patient teaching needs on admission and throughout plan of care. Arranges conferences with patient and/or family and other members of health team when indicated to plan care. Prioritizes and organizes nursing interventions that are evidence based and reflective of established standards of care. Provides individualized, mutually agreeable, patient and family education that is founded on evidence-based practice and research guidelines. Documents all assessments, interventions and patient responses. Reviews consistently the patient’s health data to keep the Plan of Care updated. Assures that revisions to the plan of care are systematic and ongoing and are driven by measurable realistic goals. Documents the patient’s progression toward identified outcomes Identifies unmet outcomes and provides direction for continuity of care. Reassesses nursing actions for effectiveness in achieving desired outcomes. Communicates with patient/family and health care team members re: patient care and nursing’s role in the provision of care. Collaborates with patient/family and healthcare team members re: the patient’s progress/plan of care. Presents relevant patient information at multidisciplinary meetings/conferences for feedback from peers. Makes appropriate referrals as needed for continuity of care. Qualifications Required: Bachelor of Science in Nursing (BSN) Current State of Illinois Registered Professional Nurse American Heart Association Basic Life Support (BLS) Strong interpersonal skills Ability to collaborate with others Strong analytic abilities Maintains active driver’s license and auto insurance Associate Degree Nursing (ADN) acceptable with commitment to obtain a BSN from an accredited program within three years of first day of employment in clinical nurse role. The Oncology Nurse Navigator directs the coordination of care for patients with Hereditary Hemorrhagic Telangiectasia (HHT) and Lymphoma patients. As an Oncology Nurse Navigator (RN) for the HHT and Lymphoma Teams, you serve as clinical liaison and the single point of contact for patients, coordinating their care as they move from initial diagnosis, treatment, and survivorship. You help ensure timely delivery of care and eliminate potential barriers, identify resources, and serve to enhance patient care and the patient experience. In this role, the Oncology Nurse Navigator is responsible for the coordination of all aspects of patient care for this population including education, multidisciplinary referrals, data tracking, diagnostics, determining protocol eligibility, treatment orders, consents and other activities essential to the overall care of the patient. Participates in the conduct of clinical research trials including multi-center cooperative group and intergroup trials, multi-institutional pharmaceutical trials (Phase I, Phase II/III trials), and in-house, Investigator-Initiated trials designed, implemented and coordinated in the Hematology/Oncology Section. Serves as liaison between Attending Physicians, Advanced Practice Providers, Primary Investigators, sponsors, patient and family members, clinical research coordinators, referring physicians and community providers, pharmacists, social workers, financial specialists and other members of the multidisciplinary team. Teach patients and their family members on injections, signs and symptoms, and medication side effects. Coordinates and conducts patient and family education utilizing NCODA or HHT Cares resources. Participate in the development of educational resources and materials for patient and family teaching. 10% --Process Improvement. Participate in process improvement projects to address lymphoma or HHT operations. Accountable to actively participate in task forces and meetings dedicated to improving communication among team members. Participates in policy and procedure reviews and provides feedback. Accountable for adhering to program policy and procedures and other regulatory agencies. Required Qualifications Must have a Bachelor’s degree in Nursing from an accredited academic body. Must be a licensed Registered Nurse with a valid, unrestricted Illinois State license. Must have a minimum two years of clinical experience working as an RN in Hematology or Oncology Specialty.
