We are seeking a Telephonic Nurse Case Manager with compact and NY RN license to work Monday - Friday, 8 AM to 5 PM Eastern time zone. CCM strongly preferred. The Telephonic Case Manager provides remote medical case management services to injured individuals, many of whom were industrially injured. A Telephonic Case Manager assists consumers in their recovery so that they may return to the highest level of function as possible. Medical services are coordinated and assessed frequently, vocational options are explored with the injured worker and their employers, and pre-injury employment or alternate employment is secured. The Telephonic Case Manager works with all parties to the claim including: the injured person, the claim’s examiner, employer, attorneys (plaintiff and defense) and medical providers. Description The Clinical Program Manager for ALIGN-CARE Geriatric Surgery Co-Management Program is responsible for overseeing the continuity of patient care. Patients will be 65 years or older, frail, community-based, considering surgery, assessing surgical risks and provide care plans to optimize health prior to surgery. Patients enrolled in the program may come from any part of Mount Sinai Health System; however, hospitals of care receipt will be both Mount Sinai Hospital and Mount Sinai Morningside Hospital. Responsibilities Oversees clinical operations within the program, using data and fact-based problem-solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes or recommends changes to superiors as necessary. Administer IV fluids, provided completion of annual competency IV therapy training. 6. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. 7. Makes recommendations in the application of the nursing process. 8. Participates in planning, and administers direct patient care, including activities of daily living (ADL). 9. Performs point of care testing including blood glucose. 10. Performs nursing procedures as defined in the nursing procedure manual. 11. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. 12. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. 13. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Overview The Case Manager has a day-to-day responsibility for assessing the medical necessity of patient admissions, and continued stays, based on the clinical documentation in the Electronic Health Record (EHR) in accordance with Milliman Care Guidelines (MCG). The Case Manager assists in coordination of care among multidisciplinary team members such as Medical Attending’s, Residents, Interns, Specialists, Social Workers, Nursing, Dietary, Finance, and Clerical staff. Case managers identify triggers to re-admissions and needed community/social/financial support to maintain appropriate and safe discharges related to ongoing medical management of patient care needs. The goals of the Case Management services are to identify Physician and staff documentation opportunities to support Quality and Pay for Performance indicators and execute prudent and sequential care by bringing about awareness to all treatment team members on length of stay reduction, and advocacy for compliance to plan of care.
