Job Responsibilities · Personal care: Assist clients with daily living activities such as bathing, dressing, grooming, and toileting. Meal preparation: Prepare and serve nutritious meals based on dietary restrictions or preferences. Mobility assistance: Help clients move safely within their homes or between locations, including transferring from beds, wheelchairs, or vehicles. Household tasks: Perform light housekeeping duties, such as laundry, cleaning, and organizing. Companionship: Provide emotional support and engage clients in meaningful activities, such as conversation, hobbies, or light exercise. Monitoring: Observe and report changes in clients’ physical or mental health to family members or supervisors promptly. Documentation: Maintain accurate records of care provided and daily activities Required Skills and Experience Maintain punctuality by arriving on time for all scheduled shifts and provide accurate documentation of daily care activities in client records. Strong communication and interpersonal skills to connect with clients and their families. Physical stamina to assist clients with mobility and other physical tasks. Ability to maintain confidentiality and adhere to HIPAA regulations. Demonstrate reliability by consistently arriving on time and completing assigned tasks with precision. Exhibit adaptability and problem-solving skills to address clients’ evolving needs. Uphold the values of the agency by providing care with empathy, professionalism, and integrity. · Ability to use a mobile device for clocking in/out and service documentations. Qualifications Must be 18 years of age or older Valid driver’s license and reliable transportation (for roles requiring travel). Able to pass a Criminal background check as well as status report on Abuse Registry and Sexual Offender Registry . We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance. Key Responsibilities: Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes. Document clinical findings accurately and attest F2F encounters in patient medical records. Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements. Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians. Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families. Provide clinical recommendations to the IDG based on F2F assessment findings. Conduct assessments in the patient’s place of residence, including home, skilled nursing, or other settings. Notify the hospice agency promptly if unable to complete F2F visits as scheduled. May provide F2F coverage for multiple provider numbers with approval.
