Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Job Description The Opportunity : We are hiring an APP in our NSICU at the Plaza. We are looking for an APP that is hardworking and has a passion for critical care. This is a full-time night shift position. This position works closely in a collaborative relationship with the Neuro Critical Care physicians. A positive attitude and working well with all members of the multi-disciplinary team is crucial. ICU background is preferred, new grads will be considered. The level of autonomy increases with experience. The physicians expect to be highly engaged in patient care. The expectation is the APP will work towards being independently credentialed to place radial arterial lines. The APPs work closely with the Neurologists, Neurosurgeons and NSICU intensivist team, nursing staff and pharmacy. The Schedule: The schedule is 3 night shifts per week. Hours are from 5:00 pm to 6:00 am. Some shifts are longer depending on how busy the unit is. Expectations are one weekend every three weeks but could require more due to staffing needs. Approximately two Holidays per year are expected. APP will cover the ICU 24 hours per day, 365 days a year. Applicants must be prepared to work undesirable shifts. The unit is 18 beds. Staffing ranges from 1 to 2 APPs on night shifts. Orientation for new graduates is approximately 5 months and they will be enrolled in the Fellowship program. You will make hospice visits while on call during the week and will help on weekends by making home care and hospice visits. You will then be off for the following seven days so you will enjoy 26 weeks of time off per year with the opportunity to pick up extra visits during your off weeks if you desire. Job Description Saint Luke's East is looking to add to the Medical Intermediate team! Schedule: Full Time, Nights (3, 12 hour shifts) Saint Luke’s East Hospital in Lee’s Summit is looking for a Registered Nurse to join our Medical Intermediate team. Medical intermediate is a 30-bed unit that cares for a variety of diagnosis & complex medical conditions including the care of surgical, cardiac, & stroke patients. The nursing team on medical intermediate values unit teamwork, quality patient care, & has a passion for growth/development. With several experienced charge nurses who joined the team upon the units opening, this group has a strong foundation & are eagerly looking forward to growing our team. Accountable for utilizing the nursing process. Responsible for performing patient care delegating patient care tasks and for supervision of other patient care staff. Responsible for coordinating the plan of care for a group of assigned patients and consulting with other members of the health care team when indicated to ensure optimal patient outcomes. This clinical nurse demonstrates proficiency and abides by policies rules guidelines and procedures.
