As the RN Case Manager for Maternity and/or NICU, you will have a panel of members with high-risk pregnancies and/or NICU babies with complex medical/behavioral needs. You will provide support and address barriers to care and SDOH with the goal to improve health outcomes. To promote healthy pregnancy, delivery, and postpartum outcomes for both mother and baby, it is essential to provide evidence-based, trimester-specific, and postpartum education. This includes assistance in developing a birth plan, accessing value-added benefits, and understanding covered services, including Medicaid postpartum coverage. Additionally, support should be given for accessing contraceptive care based on family planning needs, conducting mental health and substance use disorder (SUD) screenings with referrals to treatment when necessary, and coordinating with obstetric and other providers for appointment assistance, communication, and advocacy. This is a Field-Based Role with a Home-Based Office. If you reside within the Wyandotte County, KS or surrounding areas, you will have the flexibility to work in both the field as well as telecommute* as you take on some tough challenges.
