SUMMARY: SUMMARY: Under the direction of leadership, the Nurse Navigator provides individualized clinical care coordination for patients with complex, chronic, or specialty specific healthcare needs across the continuum of care. Working collaboratively with patients, families, providers, and interdisciplinary teams, the Nurse Navigator identifies barriers to care, develops patient-centered navigations plans, coordinates services, promotes self-management, and facilitates timely access to appropriate care to improve clinical outcomes and patient experience.
