Functioning in lieu of or alongside of a physician, it is essential for an urgent care Advanced Practitioner to have excellent assessment skills in order to recognize more serious conditions and direct the patient to the emergency room for advanced care when necessary. This role ensures that our hospice patients and their families receive compassionate, highquality care in accordance with our mission and regulatory standards. The Supervisor supports clinical staff (RNs, LPNs/LVNs, hospice aides, social workers, bereavement staff) in implementing interdisciplinary care plans, managing staffing and scheduling, monitoring quality and compliance, and fostering a culture of teamwork and continuous improvement. Key Responsibilities Supervise, mentor, and support clinical staff across home care, assisted living, and skilled nursing facility settings. Ensure timely assessments, plan of care development, and interdisciplinary team (IDT) meetings for hospice patients. Monitor and ensure compliance with state and federal regulations (including Medicare hospice benefit requirements, documentation, licensure) as well as internal policies and accreditation standards.
