Observe residents’ physical and emotional status; provide companionship and emotional support; alert supervising nurse to any changes or concerns. Communicate with residents, families/POAs, and the care team about resident needs, status, and any observed changes. Follow all facility policies including infection control, safety procedures, and resident confidentiality. QUALIFICATIONS: Valid Connecticut CNA certification. Ability to read, write, speak, and understand English. Compassionate, resident-centered attitude; strong interpersonal and communication skills. Physical stamina and ability to lift, turn, move, and assist residents — including bending, standing, walking, and some heavy lifting — as required by resident care. Job Description RN WEEKEND WARRIORS Registered Nurse Working Supervisor Rate: $60/ hour includes shift differentials Available Shifts: 7am-7pm Every Saturday and Sunday 7pm-7am Every Saturday and Sunday The RN Nurse Supervisor plans, controls and executes the nursing services required by the residents. Supervises the day-to-day nursing activities of the facility in accordance with local, state, and federal requirements, to ensure that the residents receive the highest quality of care at all times. Together with the Nursing Director, the RN Registered Nurse Supervisor plans the operation of nursing services and the provision of emergency support requirements for tour of duty. Plans the coordination of nursing activities in line with medical needs, physician instructions and the general nursing care plan. Coordinates the activities of charge nurses to ensure proper care of residents. In conjunction with medical staff, adjusts the care plan to meet emergency medical needs. In the absence of superiors, the RN Registered Nurse Supervisor assumes total responsibility for operating decisions required by the circumstances of the moment. Takes responsibility for management of emergency situations as the need develops. Ensures presence of appropriate relief staff to assume responsibility prior to leaving the facility. Ensures notification of resident’s personal physician if there is a significant change in the condition of the resident or if the resident requires immediate medical care. Qualifications: The RN Registered Nurse Supervisor must hold a current state RN license and be a Registered Nurse in good standing. Must have previous experience in a supervisory capacity. Attention to detail, good follow through skills and ability to prioritize multiple tasks. The RN Registered Nurse Supervisor demonstrates cooperation in assisting others and ability to work as a team. Long term care experience preferred. Must be knowledgeable of general rehabilitative and restorative nursing and medical practices, procedures, laws, regulations and guidelines governing long term care. MDS Coordinator

MDS Coordinator The primary role of the MDS Coordinator is to coordinate and guide the overall process and tracking of all Medicare/Medicaid case-mix documents, and care assessments in order to assure appropriate reimbursement for services. The position is also responsible for completing concurrent MDS to assure achievement of maximum allowable RUG categories, as well as integrating information from Nursing, Dietary, Social Services, and Rehabilitation departments for appropriate reimbursement. Responsibilities: Coordinates the assessment, planning, implementation and evaluation of resident care by following the MDS process. Coordinates and facilitates care conferences with the interdisciplinary team. Coordinates, completes and reviews Minimum Data Sets (MDS) Educates, trains and assists nursing staff and other departments regarding the roles of assessments, planning, implementation and MDS process; and completion of applicable sections of MDS. Responsible for ensuring all current MDS/PPS updates are implemented and staff is in-serviced. Will cross train between PPS and OBRA. Conducts regular resident interviews as required by the MDS Available to respond to clinical emergencies, regulatory agency visits, surveys and issues outside of normal work hours as needed Qualifications: Active RN or LPN license in the State of Connecticut, required. Knowledge of Quality Assurance/Improvement processes, required. Bachelors or Associate degree in a health care related field, preferred. Experience in a supervisory position in long-term care, preferred.
