Responsible for coordinating direct admissions and hospital transfers to and from community hospitals with the intent to transfer each patient to an appropriate Tenet hospital. Other related duties as assigned Responsibilities Collaborates with clinical staff, medical staff and leadership teams to identify, trend and report barriers to patient access and throughput. Works directly with staff at each client facility to ensure timely access, appropriate bed assignment, and accurate patient information documentation to assist with an appropriate final destination plan of care at receiving facility. Utilizes applicable system(s) to facilitate bed flow and expedite patient placement within the Baptist Health System. Coordinates the transfer of patients between hospitals within Baptist Health System, as well as from medical facilities outside of the system Utilizes clinical skills to make decisions regarding placement of patients within the Baptist Health System based on patient need and physician request, in collaboration with house officers, nursing units, administrators on-call and hospital staff. Maintains awareness of current bed availability at all Baptist Health System facilities on an ongoing basis Maintains emergency room call roster by updating physician information and changing on-call physicians. Completes physician Midas reports in a timely manner and notifies risk management if necessary. Updates current bed numbers in WebEOC when necessary.
