This role focuses on operations oversight, regulatory compliance, staff leadership, financial management/controls, quality improvement and growth and outreach. The director will be responsible for supporting accreditation efforts along with survey prep and training along with plan of corrections post-survey. The director will also support education and quality improvement through partnership with key stakeholders and experts across the Hospice organization. The director will be relevant on CMS updates, changes and new guidance and ensure that the company infrastructure supports the advancements. Territory/Location: Remote, preferred San Diego, CA based. Must be willing to travel 50% to any of our west coast locations. Responsibilities: Maintains knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services Ensures compliance with the Medicare conditions of participation and other state regulations govern the provision of healthcare. Complies with all Health Insurance Portability and Accountability Act (HIPAA) requirements in accordance with federal, state and organizational policies. Participates in organizational monitoring of the quality of medical services and quality improvement initiatives. Hires, trains, mentors and develops staff and interdisciplinary teams in Hospice Supports patient care standards and key resource deployment across the organization. Supports Hospice educational and training program and strategy for the organization (including implementation and planning.) Assumes responsibility for personal growth. Develops, maintains and upgrades professional knowledge and practice skills through attendance at seminars, conferences and participation in continuing education and in-service classes. Fulfills the obligation of requested and/or accepted assignments. Demonstrate knowledge in communication and counseling patient/family in dealing with end-of-life issues Qualifications: Bachelors in Nursing or Related Field or Industry Clinical operational experience including regulatory, quality and education Proficiency in HOPE tool and related review, actions and planning aligned to CMS guidelines. Excellent written and oral communication skills required. Ability to handle stressful situations/deadlines. Strong computer and software skills with knowledge of Home Care Home Base highly preferred. Self-starter, goal directed with ability to work independently and stay focused without on-site supervision. Provides direct patient care within the scope of practice. Several disciplines collaborate to provide prescribed oncology care including chemotherapy and biotherapy. The unit is also designated as the overflow for Blood and Marrow Transplant patients. The nursing team is dedicated to care for patients who need aggressive symptom management. They work closely with a multi-disciplinary palliative care team, implementing a holistic approach to physical, psychosocial, emotional and spiritual well-being. Autonomous nursing care is the ability of a nurse to assess and provide nursing actions as appropriate for patient care populations based on competence, professional expertise, and knowledge. The nurse is expected to practice autonomously consistent with ANA Standards of Practice and Professional Performance. Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. Description Kindred Hospital San Diego is a 70-bed long-term acute care hospital offering the same in-depth care you would receive in a traditional hospital, but for an extended recovery period. We partner with your physician and offer 24-hour clinical care seven days a week so you can start your journey to wellness. For those who need special care, we offer an eight-bed ICU and two negative pressure rooms. We are located just outside of downtown San Diego, less than two miles north of the San Diego Zoo. Job Summary Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. Develops nursing care plans in coordination with patient, family and interdisciplinary staff as necessary. Communicates changes in patient’s clinical condition with Physicians, Nursing Supervisor/Manager, and co-workers as appropriate. Participates in discharge planning process. Essential Functions Maintains the standard of nursing care and implements policies and procedures of the hospital and nursing department. Directs, supervises, provides and evaluates nursing care provided to patients. Assigns or delegates tasks based on the needs and condition of the patient, potential for harm, complexity of the task, and within scope of practice of the staff to whom the task is delegated. Assigns nursing care team members in accordance with patient needs, team member’s capabilities and qualifications. Documents patient admission assessment and reassessments, patient care plans and other pertinent information, completely in the patient’s medical record according to nursing standards and policies. Performs assessment on all patients on admission and reassessments as per policy. Makes referrals to other disciplines based on assessment. Develops nursing care plan of assigned patient on admission, updates plan of care as needed and ensures plan of care is coordinated with patient, family, and other members of the team. Assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Assists physicians with examinations, treatments and special procedures and performs services requiring technical and manual skills within scope of practice. Performs treatments and provides services to level of licensure. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians, and other healthcare providers. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Interprets data about the patient’s status to identify each patient’s age specific needs and provide care needed by the patient group. Job Summary: Under direct supervision of an RN or provider performs routine patient care, excluding administration of IV medications and venipuncture, following established policies, procedures and standards; follows Quality of Service Standards; follows Universal Precautions. Essential Responsibilities: May perform nursing care functions such as vital signs, removal and/or application of dressings for assessment by licensed provider. Cleaning or irrigating wound after providers assessment and instructions, shaving and skin preparation, ear washes, NG insertion & lavage, urinary catheter insertion & irrigation. Reinforce standardized self care procedures as directed by licensed provider. Recognizes and responds to acute emergency situation following established procedures. Orients patients to system policies and practices. Provides appropriate instruction sheets based upon provider direction. Reviews instruction sheet with patient. Administers medications, excluding IV medications, as ordered by provider. Observes and documents patients response to medications and therapy. May perform tuberculin, coccidioidin and histoplasmin skin tests, provides such tests are part of a tuberculosis control program. May perform immunizations provides they are based upon written protocols. Completes necessary arrangements or organizes treatment rooms prior to surgical and/or examination procedures by setting up or laying out instruments, drapes and sponges contained in sterile trays and assembling equipment items as required for particular procedures as directed. May assist licensed provider with patient examinations and treatments by positioning, restraining patients, and assisting as directed. Provides assistance in treatment rooms during minor surgical procedures and/or various diagnostic examinations by positioning and draping. May hold clamps or retractors, sponge vascular areas, and pass instruments as specifically directed by the physician. Assures safety of patient during procedures. May prepare and transport items such as medical equipment and laboratory specimens. May clean and disinfect instruments/equipment according to standardized procedures. Follows Universal Precautions. Reports incidents of failure to follow procedures to Department Administrator or designee. Maintains departmental files, performing duties such as pulling file and assembling forms prior to appointment, and filing returned diagnostic reports as directed. Initiates and/or completes diagnostic rest forms per provider order, telephones prescriptions to pharmacies, relays medical information from physician to patient, and relays information from patient to physician. Charts in patients medical record as indicated. May perform lab tests including urine dip-tests, acetone tests, hemoccult tests, tests to determine. PH, protein, PKU. May perform hearing and vision tests, glaucoma tests and visual acuities. Collects, by non-invasive and invasive techniques, and preserves specimens for testing, including urine, and sputum and stool. Participates in Department Quality Assurance activities.
