Serenity Hospice is looking to add a dedicated and compassionate RN Case Manager to delivering exceptional care, comfort and support to patients. They will develop plans for individual needs, perform regular check-ups and communicate with family members as needed. This position will oversee the Union County areas. Responsibilities: Complete an initial assessment of patient and family to determine hospice care needs Regularly re-evaluates the patients nursing needs Use assessment data to determine care plan needs Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated RN counsels the patient and family in meeting end of life needs Registered Nurse initiates appropriate preventative nursing procedures Communicates with the physician regarding patient's needs and reports any changes in the patient's condition Supervises the Hospice Aides and LPN's Requirements: Current NJ RN License in good standing Hospice Experience Preferred Willing to travel to the assigned areas Must have reliable transportation with liability insurance Strong communication and interpersonal skills. JOB DUTIES AND RESPONSIBILITIES: Screens selected medical records in accordance with contractual agreements and departmental policies for appropriateness of admission; performing initial, continued stay, and retrospective reviews if applicable. Submits initial reviews and updates using established criteria and communicates with payers as appropriate, using established processes. Utilize established processes with the Physician Advisors to manage second level and peer to peer reviews. Ensure patient status and levels of care are appropriate on admission and prior to discharge. Makes timely contact with payers and provides information as appropriate. Documents all utilization review outcomes and activities appropriately in the medical record. Complies with all applicable payer, state and federal regulations as well as The Joint Commission requirements regarding Care Management and Utilization Review processes. Acts as resource for and provides updates to the care management staff and care team for issues related to utilization review processes. Engages providers with concerns regarding medical necessity and appropriateness of services. Escalate concerns related to medical necessity and appropriateness of services to the Physician Advisors. Works collaboratively with the Denials Specialist and Physician Advisors to manage retrospective appeals and documents according to established policy. Works collaboratively with the business office/finance to ensure proper reimbursement. PHYSICAL AND SENSORY REQUIREMENTS: Sitting for one to two hours at a time. Standing for up to 2 hours per day, up to 10 minutes at a time. Walk on all surfaces for up to 3 hours per day, and climb stairs. Must be capable of driving a car. Fingering and handling objects frequently, uses fingers to turn pages. Frequently uses hands to write and/or type. Rarely uses hands or fingers for firm grasping or twisting/turning. Join Our Team as an RN Supervisor for PDN Services per diem. Pay per visit. Summit Home Care i s a reputable, family-owned, and operated home care agency dedicated to providing compassionate care to our clients. We are currently seeking experienced and compassionate supervisory nurses to join our team of dedicated professionals. Pay per visit per diem openings. RN Supervisor Responsibil ities: Collaborate with the Patient Coordinator and Clinical Manager to ensure accurate clinical documentation for reassessments and admissions. Initiate and revise the plan of care, medical orders, Medication Administration Records (MAR), and Treatment Administration Records (TAR) as needed. Conduct monthly client visits both in-home and telephonic as required. Provide in-home supervision every 60 days or as necessary. Participate in formal and informal problem-solving processes to address EMR computer education needs and achieve established outcomes for users. Act as a liaison between Summit and the community, clients, facilities, etc. Evaluate the outcome of care and provide support to ensure that services meet the client’s needs. Inform Managers and other staff of changes in the client’s needs. RN Supervisor Requirements: Current RN license in the state. Willing to travel up to an hour in distance. Schedule visits with clients and families before due dates. 2+ years of supervisory experience. Previous experience in Home Care. Exceptional staff training and customer service skills.
