To include, but not limited to, phlebotomy, injections (will include proficiency in calculations), suturing, and other typical office procedures. Administers patient medications per provider order and policy including appropriate dosage, route, site, and documentation. Orients, instructs, and trains assigned personnel as applicable. Provides basic patient care as ordered. Assures test results are received and relayed to patient in a timely manner. Demonstrates sensitivity to patient's comfort and privacy by using the AIDET fundamentals of communication. Documents all interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record. Promotes wellness by providing patient education materials and communicating provider’s advice and instructions. Triages patient phone calls and urgency of appointments. Ensures an adequate stock of supplies and well maintained equipment. Monitors and ensures accuracy of recording on patient medical records. Maintains all required reports, records, statistics, etc. Maintains confidentiality in matters regarding the practice, its patients, and employees. Ability to use a variety of office and medical equipment including, but not limited to, telephone, computer, fax, stethoscope, EKG machine, ear washing equipment, scales, thermometers, blood pressure cuff, and basic lab equipment. Maintains a clean and sanitary work area. Maintains equipment in clean operating condition. Performs referrals and pre-certifications as needed and where applicable. May schedule patient appointments per provider order and relay accurately to patients. Takes calls from hospitals/nursing homes and relays appropriate information to practitioner in a timely manner. Collaborates with the patient, physician, and other care team members as part of a team based approach to overall patient care. Follows policies, procedures, and safety standards. Position Summary: The LPN/LVN will provide nursing care under supervision of a registered nurse, within an assigned unit. Responsibilities Initiates admission database and history, and in collaboration with the RN implements patient and family teaching and care needs. Practices in compliance with all regulatory standards and adheres to all hospital policy and procedures in the performance of duties. Performs and prioritizes nursing care tasks. Accurately follows the nursing plan of care and pertinent medical orders in collaboration and under direction of a RN. Observes and reports nursing interventions and patient responses, and documents in a timely, concise and legible manner. Recognizes, responds, and reports changes in patient conditions to RN Team Leader. Participates in unit specific quality assurance activities and patient care conferences as requested. Demonstrates principles of infection control; collaborates with infection control nurse to ensure safety of patients, visitors, and personnel. Assists in orienting new employees or students to unit routines. Administers IV venipuncture and parenteral fluids per established hospital protocol. Performs specific advanced patient care treatments as approved to perform per hospital protocol. Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives and participates in quality improvement initiatives as requested. Performs other duties as assigned. Qualifications Minimum Education: None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority. Assists with cleaning and re-stocking examination rooms on a daily basis. Transports instruments to sterile processing as needed. Assists with chart prep and provider schedule management. Assists with provider support role as assigned. Supports through assistance with note prep, verbal order entry (per scope of practice and leadership guidelines), order proposal, and prescription refill management. Competent at reception role – includes professional scripting when answering calls, appropriate documentation of message in message pools, and real-time scheduling/rescheduling of appointments. Demonstrates competence in performing critical skills to include appropriate delivery of care according to age-specific needs of the population served. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops. Cooperates with other personnel to achieve department objectives and maintains good employee relations, interdepartmental objectives, and infection control policies. Attends meetings as required. Works with a positive attitude; supports a positive work environment including teamwork within their immediate department and outside their department; and exhibits initiative daily to progress the care for our patients and the success of the department.
