Summary: The Registered Nurse (RN) plays a critical role in executing InterCommunity mission to promote whole person heath. Under the supervision and direction of the Residential Nurse Manager, the Registered Nurse is responsible for providing alcohol and drug focused nursing assessment at admission to the program, medication administration, and ongoing client health monitoring, including treatment coordination. Essential Duties & Responsibilities: Timely administer medications to patients and monitor patients for reactions or side effects. Maintain accurate, detailed reports and records. Complete focused alcohol and drug assessments at admission to the program. Monitor, record, and report symptoms or changes in patients' conditions. Order, transcribe and reconcile medications per MD order as needed. Facilitate effective crisis intervention. Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans to include referral to higher level of care. Modify patient treatment plans as indicated by patients' responses and conditions and collaborate within a multidisciplinary team to achieve best patient outcomes. Direct or supervise less-skilled nursing or healthcare personnel. Refer patients to specialized health resources or community agencies furnishing assistance. Perform other duties as required. Schedule: Monday - Friday, 8 am - 4:30 pm *Attendance at required staff meetings and assigned trainings is now a condition of employment and will be treated as paid work time. A bachelor's degree from an accredited nursing college is required. The objective of MDS coordinators is to promote the physical and emotional wellbeing of nursing facility residents. They use a Resident Assessment Instrument (RAI) to gather information from residents and their families during initial and periodic interviews. The MDS covers such areas as the patient's mood, behavior patterns, cognitive ability, and nutrition needs. Information from these assessments helps caretakers formulate individualized care plans that include support from social services, dieticians, rehab specialists, and medical staff. MDS coordinators then implement and monitor these care plans to ensure their effectiveness. An MDS nurse is the main communicator between the insurance professionals and management officials at their facility. Follow field-safety practices for independent in-home visits and escalate clinical or safety concerns appropriately. Qualifications & Skills: Active Registered Nurse (RN) or higher licensure (e.g., Nurse Practitioner) in good standing with the Commonwealth of Massachusetts. Associate of Science degree in nursing from an accredited program is required (BSN preferred). Valid driver's license, a personal vehicle, and verifiable insurance are required. Minimum of 1 year of clinical experience in a home health setting. Minimum of 1 year of experience completing assessments is preferred. Prior experience with dual-eligible populations, Medicaid, or Medicare managed care is preferred. Assessment-related certification (for example, RAC-CT) is preferred. Comfortable working independently in members' homes and managing a daily visit schedule. Ability to document accurately and efficiently in an electronic platform via a secure virtual desktop. Understanding of Medicare and Medicaid programs and the populations served. Strong interpersonal and communication skills to engage members and families. Ability to manage multiple assessments and priorities while maintaining attention to detail. Awareness of and sensitivity to the diverse backgrounds and needs of the populations served. Sound clinical judgment, decision-making, and problem-solving skills. Basic proficiency with standard office and communication software. Working Conditions: Must reside within a commutable distance of the assigned Massachusetts territory, and generally within the territory served. This is a mobile, field-based position; regular travel to members' homes and community settings is essential and may exceed 50% of the time.
