Prefer previous experience, either professional, personal or volunteer, with the elderly population. The Personal Care Aide Duties include but are not limited to the duties listed below. Medication Assistance: Count Control Substance with outgoing PCA (sign your initials if count is correct). Report any missing controlled substances to administration. Prepare medication, bring medication to dining room during meal time, and assist residents with medication. If a resident requires medication prior to breakfast, PCA shall go to the resident's room and assist the resident with medication. If a resident requires afternoon or after dinner medication, the PCA shall assist the resident with medication from the med room. PCA must always watch residents ingest medication and properly document. PCA shall update medication administration record as needed, after Physician visits & ensure MAR & doctor's orders match PCA shall ensure that resident receives all necessary documents prior to doctor appointments. Consult and correspond with doctor offices for any necessary clarifications.. Record all pertinent information re: residents in the 24 hr. Log book. Personal Care Assistance: PCA shall assist residents in the dining room during mealtime by distributing meals to residents. PCA shall assist with cleaning the dining room after meals when required. PCA shall distribute facility provided snacks to residents during evening snack time. PCA shall assist residents with showering/bathing as needed. PCA shall assist residents with maintaining a healthy hygiene routine as needed. PCA shall assist residents by laundering residents clothing, linens, and towels as needed. PCA shall assist residents that require limited mobility assistance as needed. PCA shall answer all call bells, and check residents' whereabouts during rounds. PCA shall assist residents with operating the elevator lift. PCA shall assist residents by performing housekeeping and light maintenance tasks as needed. These tasks include but are not limited to: laundering residents clothing, linens, and towels, cleaning resident rooms, making residents’ beds, unclogging toilets, adjusting room temperature, salt areas outside of facility. POSITION OVERVIEW: The RN is responsible for the complete nursing care and performance of all routine medical procedures within the scope of the nursing practice. The RN is responsible for supervising LPN’s, AMAP and Direct Support Professionals (milieu counselors) in the performance of nursing tasks and activities. KEY ESSENTIAL FUNCTIONS •Provide direct nursing care as prescribed by the physician/Nurse Practitioner, in compliance with recognized nursing standards and State and Federal regulations. •Uses nursing process to make continuing assessments of our client’s total health needs. •Perform nursing assessments, by direct physical examination. •Develop and implement an individualized plan for nursing service. The nursing care plan must be updated at least annually or whenever there is significant change in the client’s condition. •Coordinate all acute and ongoing health services, as indicated by need and regulatory mandate. •Monitor health and behavioral status, labs and efficacy of prescribed treatments. Ensure timely communication of findings to appropriate clinical personnel. •Ensure that all medical record documentation is accurate, timely and outcome based and reflected in the nursing notes. •Provide health education to all clients and their support system. •Provide initial and ongoing, but at a minimum frequency of annually, training to staff in all nursing tasks, functions, protective and preventive health measures. •Provide training and ongoing support to staff regarding medication administration, infection control, and other training deemed appropriate. •In conjunction with the Director of Nursing, interview and determine candidacy of other RN and LPN applicants. •Responsible for supervision, discipline and scheduling of L.P.N’s staff hours. •Maintain documentation of all training curriculums, attendance and post training testing. •Must have experience and ability in working with electronic health record management, electronic care coordination and electronic health record interface with external service providers. •Active participant in the Interdisciplinary Team (IDT). •On call availability. •All other duties, as assigned. Description Registered Nurse-Multispecialty-Scarsdale, NY-Full Time-Day-Offsite The Offsite Registered Nurse is a Registered Professional Nurse who provides safe, competent, quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Assists in development, implementation, and evaluation of standards of care and professional practice in collaboration with members of the health care team and in accordance with institutional policies and regulatory agency requirements. Assesses and evaluates patient needs for, and responses to, care rendered and applies sound nursing judgment in patient care management decisions. Qualifications Bachelor of Science with a major in Nursing. Associate Degree or Diploma in Nursing hired prior to January 1995.
