Responsibilities Work Schedule: Schedule depends on clinic/department assigned Operating Room/APU: Monday - Friday 8.5 to 12.5 hour shifts between 5:30am and 11:00pm Pain Management: Monday - Friday 8.5 to 11 hour shifts between 6:30am and 6:00pm TBI/SARP: Monday - Friday 8.5 hour shifts between 7:00am and 4:00pm No Weekends, No Holidays, No Call!! Core Duties :• Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.• Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF. • Perform physical exam and health histories.• Provide health promotions, counseling, and education.• Administer medications, wound care, and numerous other personalized interventions. • Direct and supervise care provide by other healthcare professionals.• Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel. • Recognize adverse signs and symptoms and quickly react in emergency situations.• Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. • Make referral appointments and arrange specialty care as appropriate.• Perform nursing services identified in the TO.• Conduct research in support of improved practice and patient outcomes. Additional Duties vary depndent upon Clinic/Department, but may include: Perform a full range of RN duties in accordance with assignment under the task order, including: triage; patient assessment and monitoring; use of patient monitoring and treatment equipment; appropriate nursing care, procedures, and treatments; execution of physicians’ orders within the guidelines of standard nursing practice; documentation of patient care and observations; and patient education and emotional support. Assess each patient and perform triage duties as assigned. Formulate and carry out a goal directed plan of care which based on determined nursing diagnosis and patient outcomes and which is prioritized according to patient needs and available resources including time, personnel, equipment, and supplies. Evaluate effectiveness of self-care given by all health team members, and contribution of systems, environment, and instrumentation in progressing patient toward outcomes. Provide treatment and discharge instructions upon patient release. Perform assessment/data collection in an ongoing and systematic manner, focusing on physiological, psychological, and cognitive status. Provide a safe and clean environment for each patient. Ensure areas are stocked and properly equipped. Identify patient/significant other learning needs and implement appropriate measures to meet identified needs. Participate in group medical appointments including Shared Medical Appointments (SMA) and Drop in Group Medical Appointments (DIGMA) as requested by the PCM Qualifications Minimum Qualifications: • Degree: Minimum Associates degree in nursing. • Education: Graduate from an accredited college or university (ACEN or CNE) • Licensure: Current, full, active, and unrestricted license to practice as a registered nurse in any state; Possess a current AHA or ARC BLS Healthcare Provider certification; IV, medications and blood administration * Certificates: BLS and ACLS • Experience: One year of experience as an RN post-graduation, within the past 2 years; minimum 2 years experience for the Operating Room * Security: Must possess ability to pass a Government background check/security clearance

Responsibilities Work Schedule: Branch Clnics: Monday - Friday for a period of 8.5 hrs between the hours of 0645am and 5:00pm, with 30 minute lunch break. No On-Call, No Weekends, No Holidays!! Core Duties: Initiate, perform and complete assigned duties in providing care to variable patient populations. Complete assigned patient care based on the patients’ conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report and record relevant patient information. Observe, identify and respond to the patient’s needs for care, including medication, equipment-assisted care and patient/family education. Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and perform treatments according to established policies/procedures. Recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed. Perform support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient’s condition before, during, and following the procedure. Orient peers and ancillary personnel to unit policies and procedures, skills, and standards of practice. Integrate healthy work environment principles that result in improved patient healthcare delivery, education of patients and their families and employee satisfaction. Additional Duties vary dependent upon clinic/department assigned: Branch Clinics: Participate in MHP/MCMH Team huddles/meetings each day to review scheduled appointments, identify MHP/MCMH Team requirements (i.e., supplies and equipment and staff support issues related to the day’s scheduled appointments) and scheduled patient requirements (i.e., scrubbing the schedule to identify diagnostic procedures or medical readiness requirements that may be ordered or prepared before the patient appointment). Prepare patient electronic health record and other appropriate forms prior to patient’s appointment. Assist the MCMH team in managing many aspects of patient’s care: referrals, hospitalizations, ER visits, ancillary testing and other as directed to achieve optimum outcomes in safe and cost effective manner. Develop/distribute lists of community resources available to patients. Establish and maintains a relationship with consulting providers, hospitals, ERs and other frequently utilized healthcare resources to serve as a bridge across the continuum for the patient. Coordinate consult/referral/ follow-up/tracking process for the practice. Coordinate continuity of care with patient to insure compliance. Qualifications Minimum Qualifications: • Degree: Certificate • Education: Graduate from an accredited community college, junior college, college, university or vocational nursing program approved by the appropriate State agency and accredited by the Commission on Nursing Education Accreditation (CNEA) and HCW may be a graduate from a State Board of Nursing or accredited nursing program. • Certification: Possess a current AHA or ARC BLS Healthcare Provider certification • Experience: One year of practical or vocational nurse experience after graduation, and possess a minimum of 1 year full-time clinical experience within the last 2 years as a LPN/LVN. • Licensure: Current, full, active and unrestricted license as a practical or vocational nurse. * Security: Must possess ability to pass a Government background check/security clearance.
