Responsibilities Work Schedule: Monday - Friday, 9 hours between 7:00am and 5:00pm, including a one hour lunch Core Duties: • Proactively identify candidates for CM or care coordination services using analytic tools, predictive analytics, and CM registries. Referrals may also come from the PCM/interdisciplinary healthcare team.• Wounded Warrior Case Manger -This case management (CM) role is dedicated primarily to the active duty wounded, ill, and injured (WII) population, but this role offers flexible capacity to support non-WII patients based on operational demand.• Identify individuals with chronic, catastrophic, complex, high utilization, high-risk, or high-cost health issues who would benefit from CM services and notify PCM teams, in accordance with DHA-AI 6025.39.• Provide CM and/or care coordination to these patients: Assess the patients’ needs, develop personalized care plans, and facilitate access to necessary healthcare services and community resources.• Provide education to patients and their families. Advocate for the patient’s needs to ensure timely and appropriate delivery of care.• Collaborate with PCM teams on CM activities. Provides routine updates and outcome metrics on patients enrolled in CM programs to PCM teams.• Provide behavioral health (BH) CM services and follow-up communications to eligible BH patients, in collaboration with the MTF Behavioral Health Lead, IAW DHA-AI 6025.39.• Document all CM or care coordination encounters within the patient’s EHR using proper coding and data capture IAW MHS coding guidance and DHA AI 6025.39. Document patient interactions, referrals, and care plans to maintain consistency and track progress.• Maintain a current registry of all patients receiving CM services. Coordinate with the PCM team staff members about patients moving into CM care (into the registries) and graduating from primary CM care back to the team (off the registries) and document such in an EHR note.• Provide CM/care coordination training, guidance, and mentoring to MTF personnel who utilize these services in their empaneled patients.• Participate in the MTF Medical Management, Population Healthy, Quality, and care coordination meetings as well as appropriate DHA or network webinars, meetings, or work groups. Qualifications Minimum Qualifications: * Degree/Education: Bachelor’s degree of Nursing. Graduate from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN), the Commission on Collegiate Nursing Education (CCNE) Incumbent degree may be considered if lower than required. * Experience: Minimum of 24 months of full-time experience within the last 60 months working in case management, care coordination, or a related role with significant, direct responsibility for comprehensive client service planning and advocacy. Prior experience working with military or wounded warriors is highly valued but not required. * Licensure: Current, full, active, and unrestricted license to practice as a Registered Nurse. * Certification: Wounded Warrior Case Manager must be certified -OR-certification eligible by the Commission for Case Management (CCM) or American Nurses Credentialing Center (ANCC). * Life Support Certification: Possess current ARC or AHA BLS Healthcare Provider certification. * Security: Must possess ability to pass a Government background check/security clearance.

Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting. * Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF. * Maintain an awareness of responsibility and accountability for own professional practice. * Participate in continuing education to meet own professional growth. * Attend and/or comply with all annual training classes required by the Command, to include but not limited to online annual training provided by the MTF: disaster training, infection control, Sexual Harassment, Bloodborne Pathogens, Fire and Safety, Chemical, Biological, Radiological, Nuclear and Explosives (CBRNE), and all other required training. * Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. * Participate in the provision of in-service training to clinic staff members. Provide training and/or clinical direction as applicable to supporting government employe­es (e.g., hospital corpsmen, students, etc.). * Attend and participate in various meetings as directed. * Perform timely, accurate, and concise documentation of patient care. * Operate and manipulate automated systems such as CHCS, AHLTA, ADS, Essentris, MHS Genesis and Clinical Information System (CIS), participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications. * Exercise appropriate clinical delegation of tasks and duties in the direction and coordination of health care team members, patient care, and clinic activities. * Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions. Duties- Specific to Registered Nurse: * The HCW shall provide services within the scope of their competencies, the applicable duties provided in the basic contract, and the following: * Utilize available automated programs and information technology, communication and management tools for proactive patient management and to facilitate patient engagement and enhance patient experience ((i.e., AHLTA, TSWF, CHCS, Care-Point and Secure Messaging). * Communicate with patients utilizing asynchronous Secure Messaging (i.e., Relay-Health) to improve communication and facilitate care through non-traditional means. * Participate in MHP/MCMH Team huddles/meetings each day to review scheduled appointments, identify MHP/MCMH Team requirements (i.e., supplies and equipment and staff support issues related to the day’s scheduled appointments) and scheduled patient requirements (i.e., scrubbing the schedule to identify diagnostic procedures or medical requirements that may be ordered or prepared before the patient appointment). Qualifications Minimum Qualifications: * Education: Possess a Baccalaureate Degree in Nursing, or an Associate Degree in Nursing, from an accredited agency such as the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). As an alternative to the education requirements in the basic contract, the HCW may be a graduate of a state accredited professional nursing program OR a program accredited by ACEN or CCNE. * Experience: Must have at least 2 years experience. * Licensure: Current, unrestricted RN license from any state. * Life Support Certification: Current AHA or ARC BLS Certifications. * Security: Must possess ability to pass a Government background check/security clearance.

Provides notification to patients of results and adjusts the treatment plans as clinically warranted.• Teach patients and family members different ways to maintain or improve their health status.• Provides healthcare and preventative services to pediatric patients and their family members, including age specific anticipatory guidance in growth and development, nutrition, common illnesses, safety, and discipline.• Evaluates clinical practice and collaborates with health care team members as the Primary Care Manager to improve patient delivery for an empanelment of pediatric patients.• Makes appropriate referrals to general pediatrician, family practice physician, and/or specialty physician as warranted. Each HCW shall demonstrate competency as required in the contract and as follows: Familiarity with Electronic Health Records, word processing, window-based programs.• Demonstrate ability to utilize verbal and written communication to work as a member of a team employing team building concepts in both multi- and inter-disciplinary settings to accomplish excellent patient care. Must be able to utilize customer service skills in person, via phone and via secure messaging to promote positive staff and patient interactions. Uses excellent internal and external customer service skills in the provision of care when interacting with patients and all staff demonstrated through care, concern, and compassion.• Completing any forms requested by patient/parent or as required by the military (i.e., overseas clearance, etc.) Prepare records and reports as required in support of services rendered in accordance with established procedures.• Maintains documentation and data collection in accordance with DoD, DHA, local facility and other specified agency guidelines.• Attend and participate in daily clinic huddles, MTF meetings, patient care reports, patient care conferences, team conferences, professional staff conferences and other appropriate professional activities only to the extent that such attendance and participation is relative to his/her assigned cases and/or performance of services as determined by the supervisor. Qualifications Minimum Qualifications: * Degree/Education: Master of Science in Nursing (MSN), Nurse Practitioner, or Doctor of Nursing Practice (DNP), Nurse Practitioner. Graduation from a nationally accredited nursing program recognized by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). * Experience: Minimum of 24 months of full-time, dedicated pediatric medical patient care services as a Pediatric Nurse Practitioner, in a credentialed primary care setting. This experience must be in a role specifically focused on providing medical care to pediatric patients.
