Duties- Specific to LPN/LVN : * The HCW shall provide services within the scope of their competencies, the applicable duties provided in the basic contract, and the following: * Utilize available automated programs and information technology, communication and management tools for proactive patient management and to facilitate patient engagement and enhance patient experience ((i.e., AHLTA, TSWF, CHCS, Care-Point and Secure Messaging). * Communicate with patients utilizing asynchronous Secure Messaging (i.e., Relay-Health) to improve communication and facilitate care through non-traditional means. * Participate in MHP/MCMH Team huddles/meetings each day to review scheduled appointments, identify MHP/MCMH Team requirements (i.e., supplies and equipment and staff support issues related to the day’s scheduled appointments) and scheduled patient requirements (i.e., scrubbing the schedule to identify diagnostic procedures or medical requirements that may be ordered or prepared before the patient appointment). * Prepare patient electronic health record and other appropriate forms prior to patient’s appointment. * Assist the MCMH team in managing many aspects of patient’s care: referrals, hospitalizations, ER visits, ancillary testing and other as directed to achieve optimum outcomes in safe and cost effective manner. * Develop/distribute lists of community resources available to patients. * Establish and maintains a relationship with consulting providers, hospitals, ERs and other frequently utilized healthcare resources to serve as a bridge across the continuum for the patient. * Coordinate consult/referral/ follow-up/tracking process for the practice * Coordinate continuity of care with patient to insure compliance Qualifications Minimum Qualifications: * Education: Be a graduate from a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) program accredited by the Accreditation Commission for Education in Nursing (ACEN) or State Board of Nursing approved LPN/LVN training program or completion of equivalent military training that permits sitting for the State licensure examination. As an alternative to the education requirements in the basic contract, the HCW may be a graduate of a State Board of Nursing approved LPN/LVN training program OR be a graduate of a nursing program a program accredited by ACEN, OR have completed an equivalent military training program. * Experience: Possess one (1) year of clinical work experience as a full-time LPN/LVN within the preceding three (3) years. * Licensure: Current, unrestricted LPN/LVN license from any state. * Life Support Certification: Current AHA or ARC BLS Certifications. * Security: Must possess ability to pass a Government background check/security clearance.

Job Specific Position Duties: • Provides Utilization Management activities and functions by using MTF specific Quality Improvement processes to identify areas for review from data, suspected problem areas, and input from departments/services within the facility. Collaborates, coordinates with TOPA and Referral Management Director when needed in accordance with TRICARE Operations manual.• Receives and makes patient telephone calls, written, or e-mail correspondence regarding specialty clinic appointments and referrals following MTF-specific processes.• Monitors routinely referral management MHS GENESIS queue to ensure patients referrals are appointed and closed out per MTF guidelines.• Ensures Line of Duty paperwork is on file prior to authorization for all reserve and guard member referrals.• Keeps abreast of MTF and local market services and capabilities. Updates capability report as needed/directed.• Conducts referral reconciliation report as directed, identifying all open referrals and provides notification to appropriate personnel for resolution.• Monitors active duty, reserve/guard admissions, including all eligible beneficiaries, to civilian hospitals and notifies Clinical Nurse Case Manager and Patient Administration Element as required.• Serve as a liaison with headquarters (99th ABW) TRICARE regional offices, Manage Care Support Team, MTF staff and professional organizations concerning Utilization Management practices.• Collaborates with staff/departments, including, but not limited to: Executive Management, Resource Management, Medical Records, Patient Administration, Group Practice Managers, Health Care Integrator Consultants, Medical Management team, SGH (Chief of Medical Staff), Coders/Coding Auditors, Medical Management, Referral Management, TRICARE Operations, patient care teams, Quality Improvement, and the Managed Care Support Contractors.• Coordinates and participates in interdisciplinary team meetings, designated facility meetings, discharge planning meetings/rounds and Care Coordination meetings. Shares knowledge and experiences gained from own clinical practice and education relevant to nursing and utilization management.• Participates in the orientation, education, and training of other staff. May serve on committees, work groups, and task forces at the facility. Provides relevant and timely information to these groups and assists with decision-making and process improvement. Qualifications Minimum Qualifications: * Education:Must be a registered nurse who has obtained advanced practice education, training, and certification to practice independently to provide primary healthcare, having graduated from a masters or doctoral degree program with specialization as a pediatric nurse practitioner accredited by a national nursing accrediting agency recognized by the US Department of Education. * Experience: As required to meet clinical competency requirements specified in the service-specific credentialing instructions. Each contract worker requesting clinical privileges shall be required to demonstrate clinical competency within the past two years in the required clinical discipline as specified in this PWS. Anticipated roles in MHS Genesis are Ambulatory LVN/ Health Technician and Registration Clerk role to complete their tasks including complete intake form, medication refill/ renewal, referrals, booking appointments, checking-in patients, communicating with other members of the healthcare team.• Assists providers with invasive or non-invasive examinations or procedures. Performs/assists with a variety of ambulatory procedures as ordered by and under the direction of a credentialed provider.• Administers oral medications, gives subcutaneous, intradermal, and intramuscular injections to patients, and administers immunizations and nebulizer treatments as ordered after completion of required training and demonstration of competency. Makes necessary judgments prior to administration of certain medications based on practical knowledge of the physiological effect, current condition of the patient and awareness of proper dosage.• Record medications administered. Reports and record any reaction to medication and/or treatment.• Performs venipuncture for blood draws and intravenous (IV) access after completion of required training and demonstration of competency. Monitors and discontinues IV therapy as directed.• Monitors patients undergoing procedures and identifies patients in acute distress or with potential for acuity change. Additional Duties for Orthopedic: • The Medical Assistant (MA) will function as a clinical support team member embedded within the Maxillofacial Surgery service and the Center of Excellence in Maxillofacial Restoration.
