Registered Nurse - Outpatient Hospice (Case Manager)

Full-time Outpatient Hospice opportunity. Prefer prior Hospice or Home Health experience $5,000 sign on Bonus with 2+ years related experience. Adult and pediatric patients are cared for as outpatients in their place of residence with access to a hospice nurse 24/7.We provide a care team that includes RNs, home health aides, social workers, chaplains and providers that provide personal care to patients and complimentary therapy with massage. Additionally we have comprehensive volunteer services and a bereavement program.We also have a pediatric hospice program and provide hospice services in the Benson area. Summary Coordinates patient care planning with other disciplines utilizing the principles of the nursing process and critical thinking skills in order to maintain patient safety and well-being. Essential Functions Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure safe patient care. Notifies appropriate physician regarding any unusual or unexpected events or problems requiring direct physician consultation. Works with other members of the interdisciplinary patient care team, implements and reinforces patient instructional needs with patient and family offering informational material and pointing out special areas of concern. Evaluates outcomes of patient care, consults with other specialists as needed and adjusts nursing care processes as necessary to ensure optimal patient care. Identifies and resolves workplace health and safety issues. P rovides appropriate, direct patient care utilizing appropriate aseptic techniques. May manage IV lines, ensure that sterile techniques are adhered to, and oversee patient movement and positioning to ensure that the patient is handled in a medically safe manner. Registered Nurse Clinical Documentation Integrity Specialist - Inpatient

Summary Provides concurrent review of the clinical documentation in the medical record; review the medical record with a clinical lens to identify any missing or understated diagnoses. Queries the medical staff when necessary, by written and/or verbal communication to obtain accurate and complete physician documentation that supports the patient condition(s) and treatment plan. Performs a thorough chart review to determine the appropriate principal diagnosis of the patient. Coordinates with coding/HIM/UR and other departments to achieve a record that reflects the acuity of the patient and level of care provided. Essential Functions Conducts initial and follow-up concurrent reviews on inpatient admissions for opportunities to clarify documentation in the medical record for accurate reflection of the acuity of the patient and justifying the level of care. Registered Nurse Clinical Educator - Ortho

Summary The Orthopedic unit is located on the 4th floor of TMC’s tower. It is a 40-bed post-surgical inpatient unit. All of the rooms are private rooms with picturesque views. The majority of the patients we care for have elected to have their surgery: hip, knee, shoulder, spine, or ankle. The patients admitted to Ortho post surgically are overall healthy, motivated to recover, and progress rapidly. Elective hips and knees typically discharge next day, and spines stay with our team two to three days. We work collaboratively with our Orthopedic surgeons, hospitalists, case management team, and therapy team to ensure patient safety and satisfaction as well as to enhance the patient experience by placing the patient in the center of our care. Provides on-going orientation and professional development for all levels of personnel within a clinical department or hospital wide.Provides clinical orientations, competency assurance and on-going education opportunities.Works in collaboration with other Clinical Educators in development and implementation of hospital-wide education programs.
