PCCU team members are ACLS certified and care for a diverse population of acutely ill patients. Some examples of the patient population are: Hemodynamically unstable patients requiring active titration of vasoactive infusions Complex diabetic patients, to include DKA, requiring continuous insulin infusions Patients with tracheostomy requiring long term ventilation Respiratory distress patients requiring heated high flow and continuous Bipap Patients that are COVID-19 positive Post-operative cardiothoracic, general and vascular surgery patients Summary Coordinates patient care planning with other disciplines utilizing the principles of the nursing process and critical thinking skills in order to maintain patient safety and well-being. (Some positions might be assigned to critical care areas requiring specialized training.) Essential Functions Provides direct patient care, evaluates outcomes, consults with other specialists as required and adjusts nursing care processes as indicated to ensure safe patient care. Writes or obtains initial nursing histories, assesses patients' conditions and develops individual care plans; maintains patient documentation. Receives and reviews preceding shift reports and floor nurse reports; makes complete rounds of all units, and notes patients' condition. Notifies appropriate physician regarding any unusual or unexpected events or problems requiring direct physician consultation. Works with other members of the interdisciplinary patient care team, implements and reinforces patient instructional needs with patient and family offering informational material and pointing out special areas of concern. Evaluates outcomes of patient care, consults with other specialists as needed and adjusts nursing care processes as necessary to ensure optimal patient care. Identifies and resolves workplace health and safety issues. P rovides appropriate, direct patient care utilizing appropriate aseptic techniques. May manage IV lines, ensure that sterile techniques are adhered to, and oversee patient movement and positioning to ensure that the patient is handled in a medically safe manner. May perform the administration of pharmacological vasodilator medications for the purpose of nuclear myocardial perfusion imaging; complies with examination competency as defined in the Imaging Services Practice Standards. Complies with policies, procedures, regulations, and standards and takes corrective action when violations occur. Orients, instructs, and educates assigned personnel. Adheres to and supports team members in exhibiting TMCH values of integrity, community, compassion, and dedication. Minimum Qualifications EDUCATION : Bachelor’s degree preferred. EXPERIENCE : One (1) year of nursing experience in an acute care setting preferred. (Positions assigned to EP: require two (2) years of EP experience or demonstrated competency in all areas of EP Lab procedures). LICENSURE OR CERTIFICATION: Current RN licensure permitting work in the State of Arizona and Basic Life Support (BLS) certification required. Additional certification may be required per department. Ensures residents are treated with respect and dignity recognizing individual needs and encouraging independence. Fosters a homelike atmosphere throughout the community. Provides direct supervision of resident care staff, which includes monitoring job performance, collaborating with community leadership on associate performance evaluations, coaching and counseling associates and conducting and documenting corrective action as needed. Coordinates training and education of resident care staff on an ongoing basis. Verifies all associate hours on time cards and weekly hours report. Completes nursing schedules to assure proper coverage to meet resident needs and according to budget. This includes arranging replacement staffing when necessary and maintaining clear records of all changes, requests and irregularities. Observes residents health status, takes actions to address concerns within scope of practice, and reports all significant changes, reactions to medications and treatment or significant incidences to Health and Wellness Director (or designee) in a timely manner. Assists the Health and Wellness Director (or designee) in medication management, ordering medication from pharmacy and maintaining records as dictated in Brookdale Senior Living policies and procedures manual. Assures that medications are properly stored, medication carts/cupboards are cleaned and medication room is organized according to standards. If within scope of practice, may administer medication to residents; may review capabilities of residents who self-administer medications on a regular basis (refer to state regulations regarding medication assistance and documentation). Monitors staff in administration of medication, to include frequents audits of medication administration records (MAR). Observes residents, records significant reactions, and notifies community leadership of resident's condition and reaction to drugs, treatments, and significant incidents. Responds promptly to all emergency calls from residents. Communicates to families regarding resident's personal needs and achievements. Reminds families of need for scheduled physician, dental, or eye appointments. Assigns vital signs to associates and monitors compliance. Assures in-house ancillary medical services such as podiatrist, doctor visits, psych visits, labs, x-ray, ambulance, etc. are scheduled and followed through. Assists in organizing and developing service plan reviews as required, with appropriate resident care team members and resident families. Participates in the maintenance of resident charts including review of documentation performed by resident assistants, monthly MAR reviews, setting up new resident and thinning records appropriately. Participates in department quality improvement activities, to include completion of quality assurance surveys as assigned by community leadership. Performs daily, weekly and monthly safety and infection control checks of the home and grounds, assisting the community leadership for resident health and safety purposes. Participates in community functions including family socials and open houses. Communicates with community professionals. Shares on-call duties as required.
