Department Overview The CHO Adult Ambulatory Oncology Infusion Clinic at OHSU primarily serves patients within Hematology Oncology, Medical Oncology and Classical Hematology. We collaborate with KCI Ambulatory Oncology Clinics, Dietitians, Pharmacy, Social Work, Case Management and other members of the multidisciplinary health care team to provide exceptional oncology care for our patients and community. Job Summary: Participates with the Health Care Team in delivering quality and customer focused care to patients in a manner which reflects Kaiser Permanentes organizational and nursing mission, vision and values. Essential Responsibilities: Coordinates post-discharge patient care needs to assure the timely and effective discharge of routine and complex patients from the hospital setting. Independently and proactively completes and documents patient assessments which are thorough, timely, age appropriate, and reflect psychosocial support systems, care needs, health plan benefits, level of care determinations for hospitalized patients. Coordinates and communicates with patients, families, and the health care team to develop mutually agreeable plans of care that optimize the use of resources to support the particular needs of individual patients. Job Summary: The self-directed Hospice and Palliative Care Public Health Nurse (HPCPHN) provides skilled nursing assessment, planning and coordination/care management across settings to patients in a private home, adult foster home, residential/assisted living care, intermediate care, skilled nursing facility, inpatient hospice facilities and hospital settings in order to maximize the comfort and health of patients and families consistent with hospice philosophy and CCS policies and procedures. Essential Responsibilities: Assume initial responsibility for assessing patient/family needs and consulting with attending physician, Hospice and Palliative Care Physician (HPCP) , other IDT staff members or other appropriate community or facility contacts as indicated by patient location and needs at the time of the assessment and care provision. Obtain data on physical, psycho/social, emotional and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. Facilitate advanced illness and/or end of life discussion with patient and family members to assist with choice of hospice or palliative care. Determine patient appropriateness for palliative care or hospice services (including hospice level of care and care site) in consultation with the HPCP. Case Management: assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process; including scheduling visits to reflect patient care needs in order to maintain continuity of the plan of care; and advocate for patient and family. Participate as an active member of the IDT to coordinate information and services to update the plan of care. Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur in accordance with CCS policies and procedures. Assure professional competency through maintaining state-of-the-art skills and knowledge consistent with state and Medicare regulations, Joint Commission accreditation standards, applicable nurse practice acts and agency policies. Participate in CCS and KP orientation and in service training for professional staff as well as agency and community programs as requested to promote professional growth and understanding of hospice and palliative care. Demonstrate familiarity with policies of the agency and rules and regulations of state and federal bodies which aid in determining policies. Job Summary: The role of the Medical Assistant I (MAI) is to work as a member of a multi-disciplinary team, performs delegated clinical and administrative duties within the supervising provider's scope of practice consistent with the MA's education training and demonstrated competency. Essential Duties and Responsibilities: Patient Flow Duties: Review daily schedule and verify availability of supplies and working equipment for the day. Prepare patient chart prior to appointment time. Direct patient flow and room turnover in assigned areas. Communicate with the patients and families and keep them informed of schedule delays. Maintain cleanliness, inventory and preparedness of assigned exam rooms. Provide assistance to patients that need help with ambulation or those dependent on a wheelchair or walker. Communicate changes to patient's schedule with the provider and team members. Coordinate interpreter services as needed. Communicate with families and patients to insure they understand flow and expectations for the visit. Provide interpretation as requested. Scheduling appointment as ordered by provider. Direct Patient Care: Perform vital sign, diagnostic tests and laboratory tests including waived lab test, venipuncture and specimen processing. Accurately document medical information in patients' records (flow sheet, progress notes, immunization records, growth charts, refill logs, etc.) lab logs and quality control logs. Determine immunization status and administer immunization as needed. Administer medication as ordered by providers. Label prepackaged medication as directed by providers. Determine type of appointment needed and prepare patient appropriately. Assist providers with procedures. Dispense information to patients as instructed by providers. Call/fax refill request to the pharmacy as ordered by providers. In-Direct Patient Care: Check sharps containers daily, replacing when full. Stock information pamphlets and forms in the clinic daily. Stock exam rooms, nurse's station, vaccine administration station and lab daily. Organize and maintain clean and soiled utility rooms and storage areas. Disinfect and sterilize equipment. Maintain proper quality monitoring/control program for sterilization system. Assist the front desk as needed to expedite check in of patients. Clean and disinfect exam tables, counters, and equipment such as scales, toys and blood pressure cuff per VGMHC Infection Control Guidelines. Assemble equipment and prepare trays for minor procedures. Schedule appointments as ordered by provider. Track patients' records as directed by provider. Clinic Expectations: Report off to covering team member when leaving the floor for break, lunch or off duty. Ability to problem solves issues in a team environment. Assist other staff as requested. Implement agreed upon changes in functions, policies and procedures. Establish appropriate patient/clinic flow priorities and follow through to completion. Ensure confidentiality of patients and staff. Demonstrates cultural competency. An enthusiastic learner with the ability to work quickly, accurately, independently, and under stressful conditions. HIPAA Requirements: The medical assistant I (MA) will have access to PHI during the course of his/her work activities. The MA I will use this information to prepare both patients and medical records for visits with the provider and to keep the flow of work going smoothly in the clinic. Applying the minimum necessary standard of HIPAA, the designated records sets to which this employee will have access include: the full medical record, and the scheduling and demographics functions of the practice management system in the view only mode. Responsibilities The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician. Essential Duties and Responsibilities Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit Takes history, examines, determines diagnoses. Provides written documentation of patient visit, per NCQA standards Takes patient vital signs, as necessary. Places case management referrals and communicates with PCP as necessary. Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis. Performs all clinical duties while observing OSHA Universal Precautions Maintains patient confidentiality Attends required meetings and in-services and participates in committees, as requested Participates in professional development activities and maintains professional licenses and affiliations Employees will be required to perform other job-related assignments as requested In this role you may work with. . . Adaptability to differing weather conditions and patients’ home/residential environments Full range of body motion including handling/lifting patients.
