Responsibilities Nurse Manager Opportunity - Inpatient Adult Behavioral Health The Pavilion Behavioral Health System includes a 110 bed inpatient hospital, a residential treatment center for youth and a residential addictions treatment program. Collaborates with the Director of Nursing the development of the unit budget necessary to carry out the patient care program; supports approved budget and goals. Ensures maintenance of appropriate staffing of the adult unit reflective of the census and patient acuity. Manages the adult unit fte’s within defined parameters. Trains and develops unit staff to meet the needs of the adult population. Assists Director of Nursing with Performance Improvement initiatives for the adult unit. Assists Director of Nursing with the development and revision of policies and procedures according to established standards of nursing care. Monitors and improves patient satisfaction scores on the adult unit. Uses an understanding of the principles of growth and development to assess each patient’s age specific needs and provide age specific treatment and care. Monitors and improves employee satisfaction scores on the adult unit. Assists Utilization Review Dept. in monitoring the lengths of stay of patients taking into consideration the patient’s clinical presentation, previous response to treatment, resources available and requirements of external regulatory agents. Develops positive rapport with the Medical Staff. Participates in committees as requested by the Director of Nursing to enhance the quality of patient care. Investigates incidents on the adult unit, submits reports to Administration, Risk Manager and other outside agencies as appropriate. Demonstrates knowledge, skills, and abilities necessary to work in an environment that provides care to adult patients. Demonstrates clear understanding of the importance of patient, visitor and staff safety. Monitors general condition of the adult unit. Notifies appropriate personnel regarding problems and potential risks to safety. Adheres to the standards and commitments of the Hospital and Departmental Service Excellence Action Plan. Collaborates with Director of Nursing to conduct regular, effective staff meetings. Assists infection control coordinator with the hospital’s infection control plan. This is in addition to our generous Paid Time Off plans! **$3.25 Weekend Differential** Overview POSITION SUMMARY: The RN Case Manager - Home Health is responsible for identifying, assessing, planning, implementing, monitoring and evaluating clinical outcomes, service utilization, and resource management for patients on their caseload. The Case Manager is responsible for a scope of care that encompasses all ages from infancy through geriatric and demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. The Case Manager collaborates with the provider to coordinate inter-disciplinary services which may include nursing, therapy, social work, home care aide, respiratory therapy, home medical equipment and home infusion services. The Case Manager is accountable for attaining the best possible patient outcomes including identification and focused oversight of patients at high risk for hospitalization. The Case Manager provides evidence-based, patient-centered care at the patient’s place of residence, using critical thinking and decision-making skills. The Case Manager has a thorough understanding of OASIS documentation and plan of care development, and their impact on 5 Star Agency Performance and Value Based Purchasing performance. Qualifications REQUIRED QUALIFICATIONS: Education: Completion of nursing program Experience: Experience as RN in a healthcare environment for two years or more. Licensure/ Certification: RN License in IL or MI depending on state they are working in. Current American Heart Association HealthCare Provider BLS is required prior to start date of employment. Valid State Driver's License with proof of active auto insurance. Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem-solving skills, with the ability to be detail oriented. Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty. Maintains a valid driver's license. We are DNV certified for our hip and knee replacement surgeries. Surgical patients on this unit includes limb trauma patients, as well low acuity medical patients. With Carle being a level trauma center most regional trauma patients will be admitted to this unit. The average length of stay for most patients is a max of three days, so staff can experience the entire recovery process of the patient.
