These are performed under the direction of the Nurse Manager, RN Supervisor, RN and/or physician in preparation for the patient’s examination and treatment. Carries out clinical activities, quality control activities, performance improvement initiatives, etc. in support of the Physician Practice as assigned. Essential Job Functions Delivers direct and indirect patient care in accordance with the LPN Practice Act, hospital and/or physician practice policies, procedures, protocols, and other professional standards of care. Collects patient health care data, including VS, weight, chief complaint and pain assessment focusing on the physical, psychosocial, age, cultural, and spiritual needs of the patient. Involves the patient/family and other health care providers when appropriate. Completes patient care treatments/testing as ordered by the physician. Administers medications/immunizations using appropriate routes and techniques specific to age of the patient. Complies with completeness, accuracy, and timeliness of documentation. Provides education and instruction, within their scope of practice, to patients and families. Assists in assessing the patient’s educational needs and identifying educational barriers. Provides educational material and/or instructions based on assessed needs. Documents all education/instruction provided including topic, individual provided to and understanding. Maintains collaborative relationship with physicians to achieve desired patient outcomes. Prepares patient for examination, treatments, and procedures. Assists the physician with treatments and procedures as requested. Provides for patient safety in compliance with hospital and physician practice policies. Utilize two forms of patient identification before implementing any form of patient care. Labels all patient specimens with identifiers per policy. Uses patient identifiers on all written documentation, including notes and point of care testing results. Assists in compliance with all regulatory agency requirements. Performs quality control testing for all point of care testing per policy. Participates in non-punitive medication reporting. Adheres to infection control process, including standard precautions, to reduce the risk of acquiring or transmitting infections. Demonstrates leadership by utilizing the concepts inherent to the practice of the professional nursing. Demonstrates an ability to solve problems independently and seeks assistance from immediate supervisor when appropriate. Maintains accountability for actions taken. Delegates patient care appropriately. Effectively functions as a resource person. Acts as a patient advocate. Performs effectively in the role of preceptor/mentor to new hires and/or students. Assists in maintaining/decreasing organizational/departmental costs. Assists in the department’s compliance with JCAHO and other regulatory agencies. Participates in continuous survey readiness for inspections and surveys. Coordinates communications between patient/families and physician practice, pharmacies, specialists, and insurance carriers. Provides advice based on protocols and physician instructions and directs patients to the appropriate level of care. Facilitates patients access to prescription refills based on physician’s order. Obtains prior authorization and/or assists with indigent programs per physician order based on the patient’s financial status or insurance coverage. Supports performance improvement initiatives with the physician practice Assists in data collection as directed by the manager/physician. Incorporates into practice performance improvement initiatives that have been implemented. Maintains the environment of care. Ensures that supplies in patient care areas are replenished daily. Consistently maintains a clean safe environment. Performs POC testing as ordered by the physician. Completes all quality control testing per policy. Identifies results that are out of acceptable range and follows up prior to performing patient testing. Uses two patient identifiers, name and DOB, prior to testing. Accurately records patient test results on appropriate result form and testing log. Prepares forms, copies records, and responds to request per policies and procedures. Assists in maintaining the patients’ medical record to facilitate filing and retrieval of information. Participates in all other office functions as necessary. Other duties as assigned. Competency and Experience [Knowledge, Skills and Abilities] Required Qualifications/Experience Graduate of an accredited School of Practical Nursing Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice. Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience. Preferred Qualifications/Experience Previous LPN experience. Consults with Denial Management Specialist, department Manager and Physician Advisor or designee to resolve issues regarding adverse determinations and denials. Assists the Denial Management Specialist in designated facets for the appeal/grievance process, including medical record review for medical necessity, conferring with Physician Advisor or designee, formulating correspondence, and maintaining accurate files. Provides timely correspondence to meet requirements of all payors, as it relates to the appeal process. Collaborates with Patient Accounting Department and other ancillary departments for resolution of payor reimbursement issues in a timely manner. Responsible for data collection related to status of denials/delays. Case Manager works in collaboration with Utilization Review Completes initial utilization review for medical necessity for an assigned patient population. Initiates assessment within 24 hours of admission or next business day. Applies Intergual criteria for severity of illness/intensity of service indicators. Makes referral to PA/VPMA or designee for second level review in cases not meeting initial medical necessity criteria screens. Recognizes and progresses the plan of care when an alternative level of care is appropriate. Conducts continued stay reviews by monitoring patient’s response to treatment and resource utilization. Collaborates with Attending Physician and healthcare team to facilitate the progression of the plan of care. Completes initial and continued stay reviews in a timely manner, in accordance with various payor contracts and guidelines. Cognizant of payor requirements for all patients in assigned caseload. Responsible for understanding and communicating plan benefit limits/availability to patient or their representative in management of case as necessary. Responsible for accurate and timely documentation in recognized data bases to support Clinical Resource Management components for each patient in assigned caseload. Identifies, track and trends Avoidable/Delay Days in Midas System. Monitors length of stay and ancillary resources use on assigned patient caseload. Required Qualifications Graduate of an accredited Nursing Program. Three (3) years of clinical experience in healthcare or recent case management experience. Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice. Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience. Proficient documentation skills. Knowledge of Payor/Insurance benefits. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of the Dept. of Health as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person in violation of the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9.
