The Clinical Case Manager maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization. Minimum Qualifications RN license and Bachelor of Science in Nursing required. 2 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required. Prior case management experience strongly preferred. This role must understand the health care continuum and have the ability to solve complex problems and possess strong communication and interpersonal skills. Must be able to work with various computer applications.
