Job Description The Family Medicine Inpatient Navigator will support the clinical and logistical needs of patients admitted to the Family Medicine Inpatient and Parent Baby Services as they transition from the hospital to home and other community settings. Despite a long history of caring for vulnerable and marginalized patients with complex chronic illness, as well as extensive efforts by our physicians, pharmacists, nurses, social work and nursing partners, we continue to have difficulty safely bridging patients to the community for continued care and mitigating unnecessary readmissions. Our aggregate 30-day readmission rate has at times been as high as 20-25%, a level well above Centers for Medicare & Medicaid Services (CMS) targets. This position helps to monitor this rate and improve it by focusing resources and support on collaborative planning and communication between patient, inpatient and outpatient clinical teams and community-based organizations. The FM Inpatient Navigator will serve patients and their designees to ensure safe discharge and re-integration into the community during and after discharge from the hospital. The navigator will be an integral part of the care team, attending daily multidisciplinary rounds and working closely with physicians, nurse practitioners, nurses, pharmacists and social workers. To learn more about the benefits of working at UCSF, including total compensation, please visit: https://ucnet.universityofcalifornia.edu/compensation-and-benefits/index.html Department Description The Pre-anesthesia (Preop) and Post-Anesthesia Care Units (PACU) at Mission Bay are comprised of 30 combined Preop and PACU beds. The team of over 30 Perianesthesia specialty nurses provide dynamic and compassionate care for a wide variety of adult patients following anesthesia. The preoperative care area prepares patients for surgery, assures their readiness, and provides preoperative teaching for patients and their families. Our postoperative area cares for patients recovering from a wide variety of surgeries and procedures for the following services: general surgery, orthopedic oncology, breast, plastics, urology, gynecology (benign and oncology), radiation oncology, head and neck, colorectal and endoscopy. The priority postoperatively is to physiologically stabilize patients, provide innovative pain management strategies, and integrate family-centered care. Once the patient is stable and awake, family members are encouraged to rejoin the patient. An interdisciplinary team of nurses, surgeons, specialty physicians, anesthesiologists, pain service physicians, and respiratory therapists work to initiate the recovery from surgery continued in the inpatient or outpatient setting. The Perianesthesia nurses work collaboratively with anesthesia providers, surgeons, and other members of the interdisciplinary team while under the support of the Unit Director and Assistant Unit Director. Required Qualifications Two years of recent (in the past five years) RN experience in Critical-Care Nursing. Previous two year inpatient PreOp/PACU experience in an academic medical center Requires On-Call commitment Knowledge of airway management, anesthesia recovery, and pain management Must be able to bend, pull and push up to 20lbs. This position requires flexibility to orient and work at all UCSF Medical Center locations.
