The RN ensures that complex medical, behavioral, and psychiatric needs are met during the transition from an inpatient setting to the community. Conduct initial and ongoing nursing assessments for individuals admitted to CRTP. Oversight of self-medication administration program, monitor therapeutic response, and document outcomes.
