Description This position works 24 hours per week with an on-call commitment 1-2 days per two- week pay period, plus a weekend commitment every 5th-6th weekend. On-call requires a 30-minute response time to the McHenry Hospital. Start time rotates between 6am and 8am with an 8, 9 or 10-hour shift. SUMMARY: The primary purpose of this job position is to assist in the planning, implementation, and evaluation of recreational, social, intellectual, emotional and spiritual programs, in accordance with the resident’s assessment and care plan, and as may be directed by your supervisors. Observe resident attendance, mood, behavior, and degree of involvement so that the facility activities & resident progress are evaluated, by noting, reporting and charting resident behavior. Assume the authority, responsibility & accountability of Activity Assistant/Aide. Ensure that all charted activity progress notes are informative and descriptive of the services provided and indicate the resident’s response to the service. Develop & maintain a good working rapport with other departments within the facility to assure that daily activities can be performed without interruption. Create & maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the facility. Assist in preparing activity rooms/areas for scheduled activity programs. Recommend to your supervisor the equipment and supply needs of the activity department. Assist in cleaning up activity areas after completing activity functions. Follow the care plan in providing daily activities for resident. Purchase department supplies, clothing and personal items for the residents. Maintain the confidentiality of all resident care information including protected health information & follow facility guidelines in reporting non-compliance with said guidelines & treat residents with dignity at all times. This individual is responsible for the daily clinical aspect functioning to meet all residents’ needs. Job Description Hourly Rate: 38.00-42.00 Responsible for medication supervision/administration in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines. May be responsible for the admissions of new residents to include all admission assessments and documentation. Responsible for completing acute and long-term care plans or service plans. May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the Director of Nursing. May be responsible for discharges, assessments, and clinical documentation as indicated. Provide general consultation regarding health-related concerns within the community. Notify physicians and/or family members of any change in resident’s health and provides proper documentation. Respond quickly to all emergencies. Maintain close communication with all departments and staff members. Maintain confidentiality of residents’ and prospective residents’ information. Understand role in the Safety & Disaster Plan. Provide educational meetings for residents and staff. Qualifications Level of Formal Education: An Associate’s Degree or equivalent from a two year college or technical school: or Two years related experience and/or training equivalent combination of education and applicable experience. Area of Study: Nursing Years of Experience: 1+ years Type of Experience: skilled, sub-acute, or memory care setting Special Certifications: Must have a Registered Nurse’s License current, with no judgments against it, in the state in which he/she is practicing and must be CPR certified Language Skills: Acceptable fluency in English in order to perform job duties and speak, read, write, and communicate with supervisors, coworkers, residents, and applicable third parties. Technical Competencies: Basic computer skills Skills and Ability: Ability to work as part of a team and handle multiple tasks safely and effectively. Position Highlights: Position: Registered Nurse - Main Location: Glenbrook Hospital Full time Hours: 6:30a to 3p M-F with Rotating Call, Wkend and Holiday schedules What you will do: The Perioperative Registered nurse provides direct patient care to pediatric, adult, and geriatric patient populations during the perioperative period, using specialized knowledge and skills to achieve desired patient outcomes. The perioperative nurse functions as patient advocate. He/she assesses the surgical environment for potential patient safety concerns and intervenes appropriately while working with the surgical team to facilitate the completion of the surgical procedure. The Perioperative Registered nurse may assume responsibility for the orientation of new staff members, educational programs, and securing and maintaining supplies and equipment necessary for the delivery of care.
