As the Registered Nurse Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient. Continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered. You'll also develop the patient's Plan of care in collaboration with other professionals and implement treatment strategies based on scientific nursing theory that promotes physical, psychosocial, emotional, and spiritual well-being. Familiar with the concepts and needs of patients/families who are facing death and dying. This is a Full Time position doing Home Visits for Hospice care Our Business Hours are Mon-Fri, 8AM-4:30PM. Oncall rotation for evening/weekends as needed Our agency covers Cumberland and Gloucester county. Candidates must be able to see patients within this area Primary Responsibilities: Makes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral Identifies the patient's/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days Initiates and coordinates the plan of care Documents problems, appropriate goals, interventions, and patient/family response to hospice care Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily Instructs and supervises the patient/family in self-care techniques when appropriate Maintains accurate and relevant clinical notes regarding the patient's condition Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Current and unrestricted RN licensure in the state of practice Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation Preferred Qualifications: 1+ years of clinical experience Current CPR certification or ability to complete within 90 days of hire Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). Position Summary : The purpose of this position is to render professional nursing care to residents; to follow facility policies and procedures to implement treatment orders; to assess resident treatment needs and take appropriate action; to maintain residents’ medical records; and to participate in supervision of care provided to residents. Experience and Education : Applied Associate, Diploma or Bachelors degree from an accredited school of nursing or institute; and one year of LTC /Skilled experience. Must have a Registered Nurse’s License current, with no judgments against it, in the state in which he/she is practicing. Must be BLS and CPR certified. A minimum of two (2) years of nursing experience, preferably in geriatrics. Position Summary : The purpose of this position is to render professional nursing care to residents; to follow facility policies and procedures to implement treatment orders; to assess resident treatment needs and take appropriate action; and to maintain residents’ medical records. Experience and Education : Current and valid New Jersey L.P.N. License with no present or prior restrictions and/or suspensions, and have not had any other professional license suspension and/or been revoked from practice. A minimum of one (1) years of nursing experience, preferably in geriatrics.
