Nurse Practitioner - Cardiology - Staten Island- Brooklyn Medical Group Clinical Patient Care : Provide comprehensive outpatient cardiology care in collaboration with a board-certified cardiologists. Perform history and physical exams, manage chronic cardiovascular conditions (e.g., hypertension, heart failure, coronary artery disease, atrial fibrillation), order and interpret diagnostic tests, and develop treatment plans. Adjust medications, provide patient education, and coordinate follow-up care. Document thoroughly in the electronic medical record. Nuclear Stress Lab Duties : Prepare patients for nuclear stress testing (exercise and pharmacologic). Explain procedures clearly and compassionately to patients, addressing questions and concerns. Administer and monitor pharmacologic stress agents (e.g., regadenoson, Adenosine) . Supervise and assist with patient monitoring during stress testing, including IV placement, vital signs, and ECG monitoring. Work closely with the attending cardiologist, nuclear medicine technologists, and nursing staff to ensure safe and efficient test completion. Assist with preliminary reporting and quality assurance as needed. This is a full time position located in Staten Island, NY. The schedule is Monday through Friday 9:00AM-5:00PM (must be flexible) One Saturday a month required. Preferred Criteria Prior experience working in an outpatient cardiology practice or stress lab. Comfort with IV starts and cardiac rhythm interpretation. Strong communication skills and ability to educate patients effectively. If not eligible/appropriate for home care services COC will indicate not admitted in the system and reason for the determination. If appropriate for home care services, the COC will complete a comprehensive assessment utilizing HCHB. In conjunction with patient’s family and physician, develops and implements the Plan of Care based upon a comprehensive physical, psycho-social and environmental assessment. Provides skilled nursing care as described in the 485-Plan of Treatment, such as, but not limited to, wound care, injections, prepour/prefill of medications, disease management, medication management, etc.. Orients and supervises home health aide personnel in accordance with regulatory requirements and documents accordingly. Evaluates the effectiveness of interventions in accordance with the plan of care. Identifies the need for evaluation by other disciplines such as physical therapy, occupational therapy, MSW, speech therapy and nutrition. Observes signs and symptoms and changes in patient’s clinical, psychological and functional status. Consults with physician regarding changes in the treatment plan. Educates, counsels, supervises patients and caregiver relating to disease management and medical regime. Documents in HCHB Clinical Software all assessments, treatments and services provided and patient response to the treatment plan.
