Hospice of Western & Central Massachusetts is seeking a compassionate and experienced Hospice RN Case Manager to join our team! As a key member of our care team, you will provide exceptional hospice care to patients and their families, building relationships and delivering comprehensive services that support their unique needs. Shift is Monday through Friday 8a-4:30p. This is an on-the-road position with mileage reimbursement. Up to $49 an hour with weekly pay. This is an on-the-road position that requires the candidate to have an insured vehicle and be willing to travel throughout the county. Weekly pay ranges from $22 to $24 an hour! 3 days a week from 8a-4:30p. You perform various services for a patient as necessary to meet the patient’s personal needs and to promote comfort. The hospice aide is responsible for observing the patient, reporting these observations, and documenting observations and care performed. Experience & Education: Must hold an active and unencumbered Certified Nursing Assistant license in this state. Must be comfortable in patients' settings (skilled nursing, hospitals, homes, etc.) Must have at least one year of experience as a CNA. Duties & Responsibilities: Performing personal care needs per plan of care, including bathing, feeding, and dressing.
