About this Job: General Summary of Position The Medical Assistant performs clinical and administrative duties in an ambulatory care setting under the supervision of a physician/provider and/or a registered nurse. Duties may include venipuncture medication and vaccine administration measurement and documentation of vital signs and assisting providers with patient exams and telephone and clerical support. Position may float to other sites/locations within the system. Primary Duties and Responsibilities Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality and safety standards. Complies with governmental regulations and accreditation standards. Assists providers with patient preparation for examination minor office procedures and with the examination and education of patients. Supports other members of the clinical care team assuring optimal best-practice patient flow; manages variable and unpredictable patient volume throughout the workday. Provides clerical support and serves as backup for the Patient Services Coordinator or other team members as needed. Participates in daily team huddles to review and prepare for scheduled patient visits. Screens all patients regardless of visit type for preventative care needs ongoing support barriers to care safety concerns and potential fall risk. Conducts surveillance for transitions of care urgent/emergent care or follow up tasks such as retrieval of documents associated with referrals/testing. Conducts motivational interviews that include patient screening and/or coaching on disease management ability motivation to change learning care needs and community outreach needs. Identifies potential behavioral health needs of patients using appropriate screening tools and notifies provider of findings. As part of a High Reliability Organization reports actual and potential problems concerning patients families and associates using established reporting protocols for MedStar Health and/or work location. Participates in performance improvement initiatives to improve care quality and safety. Ensures ongoing compliance with regulatory accreditation and quality/safety standards by maintaining and auditing temperature logs ensuring medication supplies are current/unexpired checking and maintaining emergency equipment area free of clutter and other related tasks. Participates in population management activities including an awareness of the populations served and engages the patient in self-management support care/health goal setting pre-visit planning and the capturing of information that leads to health care improvement activities. Performs selected patient and laboratory testing consistent with the scope of the practice including but not limited to Point of Care Testing tuberculosis skin testing electrocardiography pulmonary function tests and vision and hearing testing (dependent upon provider and site requirements). Removes sutures/staples after the provider has examined the patient and written the appropriate order. Documents these activities in the patient's electronic medical record (EMR) or other venues based on policy. Documents clinically related activities accurately in the patient's EMR; performs required patient follow-up: to include pap smears Pb levels HIV mammograms and STD's. Collects and processes specimens completes requisitions and documents in logbook or other applicable forms/records. Provides escort for patients and assists with patient discharge activities. Provides chaperoning for patients per policy. Fulfills infection prevention and environmental responsibilities as assigned which many include: Light cleaning during clinic hours and cleaning of equipment. Updates appropriate records of scheduled equipment maintenance and manages drug and medical supply inventories. Provides telephone screening services to gather information from patients determine appropriate disposition and document per established telephone screening protocols. Under onsite provider supervision may insert intravenous catheter for physician initiated IV fluid therapy or administration of intravenous medications. Under direct onsite physician supervision may insert remove and monitor urinary catheter for procedures and prepare and administer oral drugs and injections limited to intradermal subcutaneous and intramuscular. May perform phlebotomy. Utilizes the EMR to propose patient medication refills for provider approval. Completes requisitions for tests (if applicable). Contacts insurance companies for Pre-authorizations for patient medications and specialty referrals including insurance authorization. Floats to other sites/locations within the system as appropriate and performs all duties at multiple sites. Attends scheduled meetings including but not limited to regular staff meetings and training classes for safety infection control OSHA CLIA BLS and others as required. Nurse Practitioner – Wound & Primary Care at Home

Overview Meritus Health is building a high-autonomy house-calls program for patients who need wound care and longitudinal primary care in their homes and in our Home Health clinic. We’re looking for a Nurse Practitioner who wants to own their panel, practice at the top of license, and be the clinical backbone of this model. Role Summary In this role, you will provide in-home visits for wound care and primary care, with the potential for some clinic-based sessions at our Home Health department location. You will manage a defined panel of patients, coordinate with Home Health nurses and physicians, and have significant day-to-day independence in how you structure your schedule and care plans. Key Responsibilities Provide comprehensive in-home primary care: history, physical exam, diagnosis, and ongoing management of chronic and acute conditions. Deliver wound care in the home and clinic, including assessment, debridement (within scope), dressing selection, and application of advanced therapies. Develop and own individualized care plans for your patient panel, with an emphasis on preventing avoidable ED visits and hospitalizations. Perform periodic clinic sessions at the Home Health location for follow-up, new consults, and procedures best done in a clinic setting. Coordinate closely with Home Health nurses, PT/OT, and referring physicians to ensure seamless transitions and aligned treatment plans. Order and interpret labs, imaging, and other diagnostics; adjust treatment based on results and patient goals. Prescribe medications, wound therapies, and durable medical equipment within scope and state regulations. Educate patients and caregivers on wound care, chronic disease management, medication adherence, and preventive health. Document promptly and accurately in the EMR, including clear care plans and communication with the broader care team. Participate in case reviews/huddles with the Home Health and medical teams to review panel outcomes and refine protocols. What We’re Looking For Licensed Nurse Practitioner (Family or Adult-Gerontology) in Maryland, with prescriptive authority (or eligibility). Minimum 2–3 years of experience in primary care, home-based care, wound care, or similar high-acuity outpatient setting. Comfort working independently in the field while collaborating tightly with an interdisciplinary team. Valid driver’s license and reliable transportation for house calls within our service area. New Pay Rate: $30.24 - $45.38 Wellness RN - Full Time - Dayshift Mon - Fri 8:00am - 4:30pm Under the direction of the Physician and/or Advanced Practice Provider and the direct supervision of the Manager of Outpatient Wellness, the Wellness Registered Nurse (RN) delivers patient-centered care utilizing the nursing process, clinical judgment, and evidence-based practice to assess, plan, implement, and evaluate care for patients with actual or potential health concerns. This role is focused on promoting preventive care and wellness services across the patient population while supporting primary care practices in achieving quality outcomes, enhancing patient engagement and satisfaction, and improving population health management. The Wellness RN collaborates closely with providers and interdisciplinary care teams to identify patients with high-risk needs, develop individualized action plans, and connect patients with appropriate resources and services. Key responsibilities include: Providing preventive and wellness-focused nursing care and education. Supporting value-based care initiatives, including programs such as Maryland Primary Care Program (MDPCP). Promoting patient engagement in age-appropriate wellness activities and preventive screenings. Assisting primary care practices in improving quality metrics and population health outcomes. Educating patients on disease prevention, health maintenance, and appropriate healthcare utilization. Collaborating with physicians, advanced practice providers, and care team members to coordinate care and address identified health risks. Performing clinical duties in compliance with organizational policies, procedures, and regulatory requirements. Participating in performance improvement and quality initiatives designed to advance patient outcomes and value-based care goals. Completing other duties as assigned. Minimum Qualifications: Education - Associate Degree Nursing (ASN) or higher required. Bachelor's of Science (BSN) in Nursing or higher preferred. Experience - ASN with 6 years clinical experience or BSN with 4 years clinical experience. Licensure/Certification - Current license to practice as Registered Nurse in the State of Maryland, or if resident of a state that participates in Nurse Multi-State Compact agreement, holds Registered Nurse License from that state that is designated as "Compact" or "Multi-State Privilege" using primary source verification and provides additional documentation as required by the Maryland Board of Nursing. RN License from a compact state with a designation of "Single State" does not meet the requirement and nurse must obtain a Maryland license. Licensed to practice as a Registered Nurse in the State of Maryland. Current BLS (CPR) required in compliance with policy. Knowledge/Skills/Abilities - Ability to apply the standards of the professional nursing practice with compassion and empathy. Basic computer skills required. Must have high level of organizational and communication skills. Experience in managed care environment preferred.
