Description RN/Case Manager MSH Case Management PT Days The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. The CM actively participates in specific clinical initiatives focused on reducing the length of stay (LOS), improved efficiency, quality and resource utilization. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Participates in nursing orientation. 8. Interfaces with other departments in the Medical Center. Qualifications Licensing and Certification Requirements: Education Requirements Bachelors Degree in Nursing; Masters preferred. Experience Requirements Previous experience as in homecare, long term care or utilization review preferred. Discharge Planner or Case Manager preferred. Manager or a minimum of 3-5 years experience as a RN in an acute care setting Certification Requirements: Current NYS RN nursing license. CCM certification preferred. Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. As a member of the Medical Staff the APN demonstrates standards of professional practice that includes ethical decision making, job activities related to quality assessment/improvement, self-evaluation, leadership and research. The APN is self-directed and purposeful in seeking necessary knowledge and skills to develop and maintain competency. The APN in the Hematology/Oncology Practice utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of hematologic or oncologic diseases. The APN practice will promote wellness through conducting research, providing education and works autonomously as well as in interdisciplinary teams as resources and consultants. Responsibilities What You’ll Do The Advanced Practice Nurse (APN) is a licensed healthcare provider operating under a written collaborative agreement with a physician and is credentialed through the Medical Staff Office. The role promotes patient wellness through: Conducting and contributing to research Providing patient and staff education Qualifications What You’ll Bring Current Registered/Professional Nurse (RN) license in the State of New Jersey is required Completion of a Master’s degree in Nursing from an accredited NP/APN program Eligible for national certification as an NP/APN through an approved certifying body Demonstrated skills in: Interpersonal communication Effective verbal and written communication Creative problem-solving Minimum of two (2) years of clinical nursing experience required Current Basic Life Support (BLS) certification required Hematology/Oncology experience preferred Preferred certifications: Oncology Nursing Society (ONS) / ONCC Chemotherapy Biotherapy Certificate Radiation Therapy Certificate Oncology Certified Nurse (OCN) Participation in professional and/or specialty organizations preferred Compensation In compliance with New Jersey’s Pay Transparency Act, the salary range for this position is US/07S: $136,294- 180,084.03 per year . Operating Room Technician I, (Certified Surgical Technologist) Perioperative Services

Responsibilities The primary purpose of the Operating Room Technician I position is to be under the direct supervision of the professional nurse. The OR Technician I performs various duties to assist with surgical procedures and related operating room activities. The OR Technician I delivers care as reflected in the Association of Surgical Technologists’ Recommended Standards of Practice in a manner that is cost effective without compromising quality of care. The OR Technician I possesses knowledge of the theory and application of sterile and aseptic technique and combines the knowledge of human anatomy, surgical procedures, and implementation tolls and technologist to facilitate a physician’s performance of invasive, therapeutic, and diagnostic procedures. The OR Technician I demonstrates competency to scrub for minor/major surgical and endoscopic surgical procedures. An Operating Room Technician I is a certified technician. The OR Technician I participates in departmental quality assessment and improvement activities, meets hospital continuing education and certification requirements, participates in student teaching activities, upholds Patient Care Services philosophy and objectives, demonstrates knowledge of Association of Surgical Technologists’ Recommended Standards of Practice, AORN’s Age Specific Competency Statements and demonstrates professional growth and development. Qualifications Graduation from an accredited Surgical Technician Program. Candidates must be eligible to sit for the LCC-ST certifying examination. One (1) year of related experience preferred. Certification as an Operating Room Technician (LCC-ST) plus one (1) year of related experience preferred. Constant walking, bending, stooping and standing for long periods of time. Pushing and pulling of heavy objects such as gurneys, patient beds and OR beds. Must maintain a neat, clean, professional appearance. Must have understanding, patience, and tact in dealing with physicians and fellow workers. Must pay attention to detail. Must have excellent hand-eye coordination. Must be selfmotivated and work in an efficient and effective manner. Must work in air-conditioned environment with temperature of 65-73 degrees and humidity of 45-55%. When assigned to pediatric cases, temperature may be up to 85-90 degrees F. Must wear hospital provided scrub attire while on duty. Potential for cuts, bruises, muscle strains and exposure to contagious disease via and body fluids. Potential for exposure to anesthesia waste gases, ethylene oxide, harsh disinfectants, sterilizing agents, high steam pressure agents and radiation. Willingness to work with employees at various levels of skill, knowledge and abilities.
