Completes all required clinical documentation consistently per RHS standards which includes completing and "locking" patient notes and verbal orders, as well as completing certifications and recertification's after the department's multi-disciplinary team meetings. Completes required documentation no later than the last day of the month in order to provide timely billing. Implements/initiates a plan of care and revises when necessary, provides services and/or treatments requiring specialized nursing skill, councils the client and family in meeting nursing need, coordinates services, informs the physician and other staff of changes in the client's needs, and plans for the patient to be discharged from our services. Initiates appropriate preventive and rehabilitative nursing procedures and provides treatments and procedures that contribute to the physiological and psychosocial outcomes and stability of the patient. Participates in case conferences. Provides education to client and family. Participates in training and support of all team members, volunteers and students as needed. Smithfield, Virginia Overview Works under the supervision of Care Manager (RN or PT) to provide personal care, comfort measures and environmental services for patients (including those needing special precautions) and performs specified treatments. In addition, this position must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patient served. The individual must demonstrate knowledge of the principles of growth and development over the life span and provide the care needed as described according to the departmental policy and procedure. Promotes and maintains safe work environment for staff as well as our customers, incorporating national patient safety initiatives. Must have reliable transportation to perform assigned home care/hospice visits. The Home Health/hospice Aide may be required to work weekends and holidays as census and staffing require.
