RN For Transitional Care Unit A Transitional Care Unit (TCU) helps ease patients into dialysis. The first weeks of dialysis involve a great deal of adjustment, along with increased risk of hospitalization and mortality. As a TCU nurse, you will provide patients with a solid foundation to start their dialysis journey. The TCU nurse coordinates the care and education of patients to help them feel better and make more informed decisions about their care, including choice of treatment modality—in-center or at-home dialysis. PURPOSE AND SCOPE: The registered professional nurse Home Therapies RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. Assesses and manages patients’ response to home dialysis training and treatment therapy by following prescribed predetermined protocols and communicates patient related issues to the physician as needed. As a member of the End Stage Kidney Disease (ESKD) health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. · Performs all essential functions under the direction of the Supervisor with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse. · Performs ongoing, systematic collection and analysis of dialysis data for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Supervisor or physician as needed. · Assesses, collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Directs and provides, in collaboration with the patient, home care partner, direct and ancillary patient care staff, all aspects of the provision of safe and effective delivery of dialysis therapy to assigned patients. · Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. · Initiates or assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. · Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. · Ensures patient awareness related to transplant and treatment modality options. · Assists in the identification, evaluation, selection and education of Home Dialysis candidates and Home Partners. · Performs assessment and identifies barriers of the Home Dialysis candidate’s home environment and partner / family readiness and ability to perform dialysis treatments in the home. · Trains Home Dialysis patients and / or Home Partners on the safe, effective operation and maintenance of all Home Dialysis equipment and treatment supplies through an organized and formal Home Dialysis Training Program. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. Position Type: On-Call Wage Range: RN: $34.23 - $40.75 / hour depending on experience LPN : $28.41 - $36.29 / hour depending on experience Location: 310 Lake Blvd S, Buffalo, MN 55313 Nurse Responsibilities: Displays a courteous attitude and respect for all residents, families, and staff. Works to coordinate, oversee, and provide resident-related care. Administers medications and/or treatments in accordance with physician's orders and monitors for effectiveness/response. Manages medications, supplies, and equipment including ordering, receiving, storing, and disposing of all items in accordance with policies and procedures. Documents care using computerized medication administration and charting software. Notifies Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and takes follow up action as necessary. Monitors nursing personnel on duty during the assigned shift. Nurse Qualifications: Must be in good standing with the Minnesota Board of Nursing and have current MN Nursing license. Current BLS certification required. Must have strong communication and organizational skills. Self-starter with a desire to be a part of a team. Responsibilities Under general supervision, provides professional nursing care for patients adhering to national and company standards and guidelines for specialty regulations. Demonstrates knowledge of physiological, psychological, and developmental and age specific differences in the patient population including late adolescence, adulthood, middle age, and the elderly. Collaborates with the interdisciplinary team to provide for assessment and therapeutic management of patient/family needs. Supports and adheres to the US Oncology Compliance Program, to include the Code of Ethics and Business Standards. Prepares and administers therapies under the direction and supervision of a physician. Administers all medications/blood products using correct medication/blood product, dosage/amount, route, time and technique. Assures pre-authorization and appropriate consent forms are obtained prior to administration of therapies where applicable. Facilitates dose modification by assessing patient, assessing labs, consulting protocol or treatment plan, calculating dose, and consulting physician regarding dosage modifications, problems and symptom management Performs venipuncture, accesses implanted ports and maintains central lines catheter devices. Maintains accurate documentation of assigned patient care and treatments provided. Applies knowledge of reimbursement procedures and charge capture. Programs portable infusion pumps for continuous drug delivery and provides instruction to the patient and/or caregiver in the use of the pump at home. Demonstrates knowledge of ambulatory infusion pump use and maintenance, cleaning pumps after use, and proper documentation of pump usage. Maintains work environment Restocks the supply area as needed. Follows OSHA guidelines when handling cytotoxic agents, biohazardous waste and any other material potentially infected with a blood borne pathogen. Maintains all disinfection protocols. Maintains equipment according to quality control standards. Participates in professional development activities and maintains current knowledge of oncology nursing by participating in ongoing educational opportunities. SALARY LEVEL: $40-$60/hour (plus additional float pay) Qualifications Graduate from an accredited program for professional nursing education, BSN preferred. 3+ years nursing experience preferred, preferably at least one year in oncology RN state license and current registration with the State Board of Nursing in practicing state. Current Basic Life Support (BLS) certification. Must maintain continuing education requirements.
