Case Management Coordinator

Overview About the Role: The primary purpose of the Case Management Coordinator position is to coordinate the patient’s progress through the system. He/she collaborates with physicians and clinical staff to ensure resource utilization is appropriate to the patient’s clinical and psychosocial needs and that it remains within covered benefits. The Case Management Coordinator seeks creative solutions to process and system issues, which create a barrier to the delivery of medically appropriate, cost-effective care. He/she is responsible to ensure compliance with regulatory standards related to level of care determinations, utilization and discharge planning. The Case Management Coordinator performs transitions of care and discharge planning, and supports the mission of University Hospital. Identifies and documents proper level of care; discusses cases with attending and physician advisor; identifies anticipated length of stay; works with attending on clinical pathway or MCG guideline; proactively assists in resolving barriers; develops and implements case management plans and interventions; performs discharge planning; completes responsibilities by following established guidelines and protocols within the appropriate timeframe. Initiates the discharge plan and documents interventions; updates care plan weekly; confirms Face Sheet information; assures proper hand off for transfers; communicates discharge plan to new case manager; assesses need for referrals to outside resources; obtains authorizations for Home Care, Home IV infusion and DMEs; coordinates forms or electronic referrals; arranges transportation; meets departmental performance expectations for home care/hospice and equipment; confirms services and contact information. Identifies readmissions within 30 days; performs walking rounds and identifies high‑risk patients; works with Social Work and utilization management to reduce LOS and prevent denials; coordinates and manages distribution of IMM and assists with MOON; conducts or co‑leads care coordination rounds; promotes working relationship with patient/family; acts as advocate; works closely with Social Work and utilization management; serves as a resource regarding patient classification and reimbursement. Demonstrates competency with computer programs; achieves Department Team Dashboard Score Card goals; escalates avoidable days; completes mandatory trainings and in‑services; demonstrates knowledge of safety, security and infection control risks; participates in quality improvement by collecting and analyzing data, identifying trends and suggesting improvements; participates on clinical process improvement teams. Demonstrates commitment to excellent customer service; will rotate on weekends to provide coverage; understands and adheres to University Hospital compliance standards; keeps abreast of pertinent federal, state and hospital regulations, laws, and policies. Performs other related duties as assigned. Qualifications What You’ll Bring: Registered Nurse with a license to practice in the State of New Jersey required. Overview About the Role The primary purpose of the Critical Care Transport Team (CCTT) Staff Nurse is to safely move patients requiring PCU or ICU level of care within the University Hospital and Rutgers campus. The CCTT RN prioritizes transport requests based on acuity and workload logistics. The CCTT RN will utilize the Acuity Checklist for Critical Care Transport form to determine who will accompany the patient during transport and what equipment will be needed. The CCTT RN will require a thorough and complete SBAR (Situation, Background, Assessment and Recommendation) report, in person, from the primary care PCU/ICU RN managing the patient. The CCTT RN will complete their tasks with compassionate consideration of the patient’s requests and those of their family. The CCTT RN will utilize the Inter Transport Patient Data Form to chart the appropriate data as it applies to the individual patient’s circumstances. The CCTT Nurse takes immediate action in emergency situations and responds to patient needs during transport. Upon arrival, the CCTT RN will give a complete and thorough SBAR report to the receiving RN and will assist in the safe transfer of care. All transfers will be conducted with emphasis on preventing skin breakdown and the secure positioning of lines, tubes, and drains. Overview About the Role The primary purpose of the Staff Nurse position is to provide direct patient care, perform nursing procedures, assess, plan, implement, and evaluate care for diagnostic and interventional cardiology patients in a Cardiac Catheterization Lab setting as observed by the Nurse Manager/Assistant Nurse Manager, and patient/family feedback unless otherwise noted. Responsibilities What You’ll Do Provide direct patient care in the Cardiac Catheterization Lab. Perform nursing procedures for diagnostic and interventional cardiology patients. Incorporate patient and family feedback into care evaluation Qualifications What You’ll Bring Licensed as a Registered Nurse in the State of New Jersey. Minimum one (1) year of experience in special procedures, radiology, or Cardiac Catheterization Laboratory required. Applicants with at least one (1) year of recent experience in Critical Care, Telemetry, or Emergency Department nursing will also be considered. Current ACLS and BLS certifications required. Completion of 6–8 weeks of Cath Lab procedure training with an experienced nurse preceptor for newly hired Cath Lab nurses. Compensation (NJ Job Transparency Law) The expected hourly range for this position is XH/99H:$55.00 per hour, in accordance with the New Jersey Pay Transparency Law; final compensation will be commensurate with experience and qualifications. Overview About the Role Leverages extensive subject matter expertise on clinical systems and knowledge of clinical operations and workflows to coordinate the activities of medical staff in the planning, development, implementation, and support of clinical information systems/solutions to improve the delivery of patient care and hospital operations. Responsibilities What You’ll Do Assists physicians, nurses, and other clinicians with design, implementation, workflow optimization, and issue resolution related to electronic health record (EHR) and other clinical applications. Functions as a liaison between the clinical community and IT analysts, and a key resource for application and/or clinical workflows and software systems. Develops and manages complex projects. Works with clinical users to understand clinical information technology project needs. Recommends resource planning and support needs to ensure successful project implementation. Oversees the design and development of application programs from analysis to implementation and assists with these duties as needed. Creates system specification and workflow documents for new projects and/or features. Maintains existing specifications and project documents for complex clinical IT initiatives that include technical details, user expectations, project goals, work effort, accountability, and deliverables. Coordinates the installation of software and/or hardware. Works in concert with other HST/IS&T Departments pertaining to the planning and implementation of systems to ensure that HST/IS&T standards are met. Qualifications What You’ll Bring Required: Bachelor’s Degree, with preference for Hospital Administration, a health related discipline (Nursing, Pharmacy, etc.), or related clinical field. Licensure in a clinical profession is required. Five (5) years of experience in information systems with preference given to candidates within a hospital environment. Extensive subject matter expertise on clinical systems and knowledge of clinical operations and workflows. Direct experience with electronic medical records is a plus; prior Epic experience is highly desirable. Must complete certification on Epic application specified by management within 6 months of training. Demonstrates interpersonal skills working in a service-oriented environment. Excellent oral and written communications skills. Capable of functioning/leading in a team environment. Equivalent education, experience and/or training may be substituted for the degree requirements. Additional relevant experience, or clinical expertise, may be substituted for information systems experience. OB Scrub Technician-Family Health Services

Responsibilities The OB Scrub Technician provides direct care to obstetrical/neonatal patient under the direct supervision of a licensed RN. The OB Scrub Technician performs the role of surgical scrub for obstetrically related surgical procedures. Maintains aseptic environment during set-up for surgery or delivery and throughout surgical procedures. Responsible for maintaining and stocking supplies and equipment throughout the Labor and Delivery Suite, including Recovery and Operating Rooms. Assists with perioperative care as outlined in the policy and procedure manuals. Assist with deliveries, as necessary. Assists with other aspects of unit operations as assigned. Qualifications Graduate from an accredited Surgical Technician Program. Certified as a Certified Surgical Technician (CST) by the National Board of Surgical Technology and Surgical Assisting or equivalent required. AHA CPR required. One (1) year of related experience preferred. Must maintain a neat, clean, professional appearance. Must have understanding, patience, and tact in dealing with physicians and fellow workers. Must pay attention to detail. Must have excellent hand-eye coordination. Must be self-motivated and work in an efficient and effective manner. Must work in air-conditioned environment with temperature of 65-73 degrees and humidity of 45-55%. When assigned to pediatric cases, temperature may be up to 85-90 degrees F. Must wear hospital provided scrub attire while on duty. Potential for cuts, bruises, muscle strains and exposure to contagious disease via and body fluids. Potential for exposure to anesthesia waste gases, ethylene oxide, harsh disinfectants, sterilizing agents, high steam pressure agents and radiation.
