On an An Average Day you Will: (Includes, but not limited to) Complete an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs Provide a complete physical assessment and history of current and previous illness(es) Provide professional nursing care by utilizing all elements of the nursing process Assess and evaluate patient’s status by: Writing and initiating plan of care Regularly re-evaluating patient and family/caregiver needs Participating in revising the plan of care, as necessary Initiate the plan of care and make necessary revisions as patient status and needs change Use health assessment data to determine nursing diagnosis Develop a care plan that establishes goals, based on nursing diagnosis Include the patient and the family in the planning process Record pain/symptom management and changes/outcomes as appropriate Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders, as required Teach the patient and family/caregiver self-care techniques, as appropriate Work in concert with the interdisciplinary group All other duties as assigned Requirements: Must have at least one (1) year of acute care experience in an institutional setting One (1) year of experience in home care preferred Must have a current licensure in the state Must understand the philosophy of hospice concept and understand needs of the terminally ill Must be flexible in work hours Must demonstrate nursing skills per competency checklist Must demonstrate a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs Ability to travel throughout the Bristol Hospice Service Area We Got the Perks: Tuition Reimbursement PTO and Paid Holidays Medical, Dental, Vision, Life Insurance, and more HSA & 401(k) available Mileage Reimbursement for applicable positions Advanced training programs Passionate company culture committed to the highest standard of care in the hospice industry Join a Team that embraces the reverence of life! As a Sweet Dreams CNA You Will: (includes but not limited to) Spend quality one on one time with patients and their families to support everyday needs Care for hospice patients by providing paths, back rubs, oral hygiene, shampoos, and changing bed linen as often as needed Provide bedtime care including personal hygiene regimens as required Provide aroma therapy and gentle touch Provide lotion rubs to hands and feet Perform personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. Provide patient and family with positive communication techniques, within realm of ethical and respectful care, including confidentiality. Certified Medical Assistant - CMA

Obtains and documents, using the rooming tool in the electronic medical record, the patient’s reason for visit, weight, vital signs, visit screening information, medication review, and pertinent information for provider. Sets up examination and treatment rooms between cases. Assists patient as needed to prepare for visit with provider, takes appropriate measures to assure patient’s safety and privacy. Provides direct patient care as directed by provider based on competency and needs of clinic, such as performs EKGs, wound dressings, and functions as support to the provider during minor surgical procedures and ancillary examinations. Prepares and administers medications according to policy and procedure after validation of knowledge and demonstration of competency. May not administer intravenous medications. Completes patient tests and treatments per provider’s order according to policy and procedure after validation of knowledge and demonstration of competency. Collects and prepares laboratory specimens and completes associated paperwork accurately and efficiently to expedite laboratory studies. Performs phlebotomy and may assist in specific laboratory functions relative to the testing, reporting, and quality assurance of laboratory specimens as educated and overseen by the laboratory. Performs receptionist and clerical duties to support patient care and functioning of the clinic, including matching and filing ancillary department reports, consultation and other medical reports in patients’ medical records. Documents in the appropriate sections to create a complete and accurate medical record. Uses the electronic medical record and scheduling system as designated. Facilitates timely appointments for patients. Completes referrals and pre-authorizations as needed. Collects co-payments.
