Key Responsibilities: Provide care for adult and pediatric patients in New York City through home visits, monitoring, and treatment. Conduct assessments and treatment under APP supervision for patients with limited access to healthcare. Maintain accurate patient records using HealthPoint and Athena EMR systems. Practice with telemedicine support and ensure compliance with clinical standards. Act as a conduit for medical treatment in Health Plan Partnerships. Engage in health management, medication, and care goal discussions. Provide supervised treatment for common diagnoses. Participate in ongoing education and adhere to safety protocols. Deliver compassionate, patient-centered care. Qualifications: Minimum of two years of experience as a Licensed Practical Nurse (LPN) Active and current American Heart Association BLS certification required Experience in Emergency Department, Urgent Care, Internal Medicine, Critical Care, Family Practice, or other relevant areas preferred Active and current driver's license for 2+ years required Current unrestricted NY State LPN license required Clinical work experience with pediatrics required Phlebotomy experience required Fluent in Spanish preferred Ability to lift 50-60 lbs Apply Today!
