This division focuses on supporting individuals recovering from illness, surgery, or acute exacerbations of chronic conditions, allowing them to receive skilled care while maintaining independence in a familiar environment. Intermittent services are delivered on a part-time or as-needed basis and include skilled nursing, physical therapy, occupational therapy, speech-language pathology, and home health aide support . Care is tailored to each patient’s individualized plan, with an emphasis on restoring function, improving mobility, managing symptoms, and promoting self-care. The program primarily serves patients transitioning from hospital or post-acute settings, making it a critical component of the care continuum . Clinicians collaborate closely with physicians, case managers, and caregivers to ensure coordinated, seamless care. Frequent assessments and care plan adjustments are conducted to align with patient progress and evolving needs.
