TCAT LPN Graduates Thanks for reaching out! T.C.A 63-7-110 (d) (1) (states: (1) Notwithstanding a law to the contrary, a graduate practical nurse may engage in the practice of practical nursing without a license for a period not to exceed one hundred twenty (120) calendar days from the date of receipt of the first authorization to take the NCLEX-PN examination if: (A) The graduate practical nurse's practice occurs in a healthcare institution licensed under title 33 or 68, or an affiliate of the institution; (B) The graduate practical nurse is at all times working under the supervision of an individual licensed in this state to practice as a professional or registered nurse. The graduate practical nurse shall provide the healthcare institution or affiliate of the institution in which the nurse is practicing with the following: (i) Proof of the first authorization to take the NCLEX-PN examination; and (ii) Proof of graduation from an approved school of nursing within the previous ninety (90) days; (C) The supervising individual described in subdivision (d)(1)(B) is limited to supervising no more than one (1) graduate practical nurse at a time; and (D) The graduate practical nurse is prohibited from: (i) Being deemed a licensed practical nurse pursuant to this section; and (ii) Using another title or identifying as anything but a “ graduate practical nurse” in a clinical setting. (2) As used in this subsection (d): (A) “ Graduate practical nurse” means an individual who: (i) Holds a diploma or degree from an approved school of nursing that entitles the individual to take the NCLEX-PN licensing examination; and (ii) Has received authorization to take the NCLEX-PN examination; (B) “NCLEX-PN” means the national council licensure examination for practical nurses ; and (C) “Supervision” means that the graduate practical nurse's supervising individual is located in the same unit as the graduate practical nurse when the graduate practical nurse is performing duties pursuant to this subsection (d). Nurse Unit Manager (LPN or RN)

Unit Manager • Responsible for supervising, evaluating, counseling, and disciplining personnel on assigned units. • Responsible for ensuring continuity of care for 24 hours, 7 days a week. Receive report from previous supervisor for all nursing units, and forward report to oncoming supervisor prior to leaving tour of duty. Follow-up all identified issues. • Responsible for problem identification and resolution at the unit level. Keep the Director of Nursing/Associate Director of Nursing informed of residents' status through verbal communication and written report. • Assure that effective quality nursing care is delivered which is outcome focused through utilization of the nursing process. Identify needs and provide input for discharge planning and follow-up throughout length of stay • Be point person for admissions during off hours, including providing tours, welcoming new families and residents, ensuring a smooth and comfortable admission experience. • Implement plan of care consistently, effectively and cost efficiently with focus on resident centered outcomes • Work in collaboration with physician and/or other health care professionals by sharing information relevant to changing plan of care. • Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices. • Will supervise and coordinate Charge, Treatment and CNA assignments and activities in an assigned unit and shift. Will ensure timely communication of current treatment & assessment information, efficient delivery of services and timely compilation of new information. • Will monitor and evaluate staff activities to ensure that nursing care is carried out as directed, and treatment administered in accordance with treatment plans and physician's instructions. Qualifications/Requirements: • Must have graduated from an accredited Nursing college or university. • Must have a valid professional RN or LPN license for state of TN • Experience with long-term care and/or geriatric care is preferred but not limited to. • Well versed with documentation writing • Document patient’s progress on weekly progress note in a timely fashion. • Collaborates with all disciplines to provide best patient centered treatment and care. • Exceptional patient service skills and ability to demonstrate clinical excellence • Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times. • Compliance with confidentiality, HIPPA and Federal Residents Rights. RN (Registered Nurse) Job Duties: Assists in defining, maintaining and interpreting standards of practice. Performs required skills (assessment, intervention) according to guidelines, policies and procedures. Participates in interdisciplinary committees which influence or determine policies affecting nursing practice. Follow facility procedures/protocols for admitting residents transferring to hospitals/clinics/outpatient visits and completion of appropriate documentation based on established guideline. Makes rounds on all resident rooms and ensure that the L.P.N.’s, C.N.A.’s and/or others under his/her supervision receive their resident reports, has their assignments and break times scheduled. Assist the L.P.N. in establishing routines which facilitate a high level of nursing practice. Receives and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts. Coordinates/Prepares MDS schedules for quarterly, annual and significant change assessments. Complete MDS Booklet as per schedule (Nursing Sections). Reviews and signs off on all completed MDS and RAP Summary. Develops care plan that demonstrate knowledge of criteria to ascertain resident specific needs based on health care problem(s). Makes needed revisions and updates to the care plans. Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Other responsibilities as deemed necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. RN (Registered Nurse) Qualifications/Requirements: Graduate of an accredited School of Nursing M.S./B.S. or equivalent experience preferred RN Licensed to practice Nursing Experience which one has acquired clinical competence and strong leadership skills.
