Summary PRM LPN performs advanced practical nursing work in the care and treatment of patients in assigned area(s). Employees in this classification perform a variety of nursing care assignments in one or more medical areas, requiring more advanced knowledge and skills of practical nursing. Emphasis of the work is on performance of specialized as well as more difficult and responsible duties in patient care, giving assistance to physicians and nurses, and performing instructional tasks. Responsibilities Interviews patient about current problems, medications, and allergies and reviews medical record for pertinent previous and current information. Keeps patients informed of their medical condition and the course of treatment being prescribed by an MD. Participates in patient treatment regimen, including administering medications (under the supervision of a physician and registered nurse). Establishes and documents patient treatment regimen under supervision of RN or MD and implements treatment as directed. Performs routine immunizations, nebulizer treatments, and visual acuity testing, and administers and reads TB tests. Assists in phlebotomy: identifies patient, checks requisition form for requested tests, and prepares equipment, patient and puncture site. Assists in IV set-up: verifying solution to be administered, evaluating infusion site, checking IV function and patient comfort, and discontinuing IV at appropriate time, as ordered by MD and under supervision of an RN. Maintains adequate inventory of narcotics after each tour of duty, examination room supplies and prepares exam rooms at the beginning and end of day and between each patient contact. Performs variety of office responsibilities: answers phones and takes messages and performs triage within scope of licensed practical nursing practice in an accurate and timely manner. Facilitates and assists in the coordination of patient referrals. Participates in the orientation and training of new personnel. OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and the Nursing Policies and Procedures. As an LPN you will be responsible for providing residents assistance with ADLs and in the administration of their medications while overseeing staff. Adhering to the instructions of Nurse Management, you will perform your duties in accordance with all company policies and procedures, while providing resident care services with kindness and compassion to the residents. RESPONSIBILITIES: · Holds daily shift report with nursing assistants assigned to the unit prior to the start of the shift. Reviews written daily assignments for each resident on assigned unit. Assures assignments are completed before the end of the shift · Makes rounds on assigned residents at least once every two hours · Administers medication and treatments according to the physician's orders. Assumes responsibility for ordering medications · Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a monthly basis or more often if the resident's condition changes · Assists the attending physician with rounds and special procedures as needed · Transcribes, documents and implements physician's orders · Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering · Assumes independent responsibility for yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance.
