The Licensed Practical Nurse “LPN” provides basic care to patients, performing all job functions in an ethical manner and ensuring patient safety, privacy, dignity, and confidentiality. Job Responsibilities: The Licensed Practical Nurse (LPN) ensures that the ANA scope and standards of practice and the code of ethics for nurses are the foundation for nursing practice. Educates and prepares the patient/family for future self-management by facilitating patient movement from passive to an active partnership and eventually to a personal self-care expert. Serves as a clinical preceptor for new staff/students as appropriate. Provides shadowing experiences for prospective new employees as requested. Ensures appropriate usage/ordering of supplies and proper charging. Performs consistent with Shepherd Center standards, clinical policies, and procedures. Practices proper safety techniques in accordance with Center and departmental policies and procedures. Responsible for the reporting of employee/patient/visitor injuries or accidents, or other safety issues to the supervisor and in the occurrence notification system. Actively participates in center-wide efforts that affect nursing practice. Adheres to patient/family centered principles using the communication and engagement tools (OARS/AIDECT). Practices proper safety techniques in accordance with Center and departmental policies and procedures. General Purpose: Provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice and regulations and as directed by the Director of Nursing. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required. Monitor seriously ill residents, as necessary. Review care plans daily to confirm that appropriate care is being rendered. Confirm that CNAs are aware of the resident care plans. Verify that the CNAs refer to the resident’s care plan prior to administering daily care to the resident. Other Duties Attend in-service education programs in order to meet facility educational requirements. Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill and Evacuation Procedures and know how to use the information. Maintain confidentiality of resident and facility records/information. Protect residents from neglect, mistreatment, and abuse. Protect the personal property of the residents of the facility. Others as directed by the supervisor or administrator. Minimum Qualifications: Licensed Practical Nurse with current state license. Excellent technical, assessment and documentation skills. Good Leadership qualities. Organized and detailed in work performance. Good communication skills with excellent self-discipline and patience. Genuine caring for and interest in elderly and disabled people in a nursing facility. Comply with the Residents Rights and Facility Policies and Procedures. Responsibilities Verify plan of care with the RN prior to each visit. Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes. Teach patients and caregivers self-care techniques. Check vital signs. Review all medications and perform a medication reconciliation at each visit. Perform specialized procedures ordered by physician (i.e. foley catheter change). Prepare equipment and materials for treatments using the aseptic technique. Complete documentation timely and accurately. Qualifications One year of clinical LPN experience.
